Comparison of results of conservative method and plate fixation method for the treatment of Ruedi/allgower type 1 Pilon fractures

Abstract

Objective: The aim of this study was to compare the re­sults of cast application and plate fixation in the manage­ment of Ruedi/Allgower type I Pilon fractures Patients and methods: Forty-two patients (24 females, 18 males) with Ruedi/Allgower type I Pilon fractures were retrospectively reviewed. Sixteen patients (Group 1) (10 females, 6 males; mean age 43 years; range 18-56) had been treated with cast application and 26 patients (Group 2) (14 females, 12 males; mean age 37.7 years; range 19-52) had been treated with plate fixation. All patients were diagnosed with X-ray. Radiologic examinations were made using Ovadia and Beal\'s criteria. Long term functional results of foot and ankle were evaluated ac­cording to the Tenny and Wiss citeria. The mean follow-up period was 28 months (range 12-44) in Group 1 and 31 months (range 16-46) in Group 2. Results: Mean reduction quality score was 12 points in Group 1, and 10 points in Group 2. The difference be­tween the groups was statistically significant (p0.05). Time to union was not different between both groups (p>0.05). Conclusion: Although the quality of reduction obtained with cast application was not as satisfactory as plate fixa­tion; good results could be obtained in both groups re­garding long term functions.

Ruedi/Allgower tip 1 Pilon kırıklarının tedavisinde konservatif yöntem ile plak yöntemi sonuçlarının karşılaştırılması

Abstract

Amaç: Bu çalışmanın amacı Ruedi/Allgower Tip 1 pilon kırıklı olguların tedavisinde alçılama ile plak yöntemlerinin sonuçlarını karşılaştırmaktır. Gereç ve yöntem: Ruedi/Allgower Tip 1 pilon kırığı ne­deniyle tedavi edilen 42 hasta (24 kadın, 18 erkek) geriye dönük olarak incelendi. On altı hasta (10 kadın, 6 erkek; ortalama yaş 43 yıl; dağılım 18-56) alçı (Grup 1) ile 26 hasta (14 kadın, 12 erkek; ortalama yaş 37.7 yıl; dağılım 19-52) açık redüksiyon ve plak (Grup 2) yöntemi ile teda­vi edildi. Tüm hastalara direkt radyografi ile tanı konuldu. Hastalar erken dönemde postoperatif grafileri incelenerek Ovadia ve Beals\'ın belirlediği kırık redüksiyon kalite skor­larına göre, uzun dönemde de Teeny ve Wiss tarafından geliştirilen ayak ve ayak bileği fonksiyonel sonuçlarına göre (100 puan üzerinden) incelendi. Ortalama takip sü­resi Grup 1\'de 28 ay (12-44 ay) , Grup 2\'de 31 ay (16-46 ay) idi. Bulgular: Erken postoperatif dönemde ortalama redüksi­yon kalite skoru Grup 1\'de 12, Grup 2\'de 10 puan olarak bulundu ve her iki grup arasındaki fark istatistiksel olarak anlamlı idi (p0.05). Kırık kaynama süreleri her iki grupta farksız bulundu (p>0.05). Sonuç: Ruedi/Allgower Tip 1 pilon kırıklarının alçı ile te­davi edilmesi sonucunda elde edilen redüksiyon kalitesi, plak yöntemine göre daha yetersiz olsa da, uzun süreli fonksiyonlar bakımından her iki yöntemde de tatminkâr sonuçlar elde edilebilmektedir.
