Five year evaluation of intraoperative pathology consultations in a university hospital

Abstract

Objectives: Intraoperative pathology consultation (IOPC) is one of the most important and most difficult processes of pathology practice and the review of the results is accepted as a quality-control method in pathology laboratories. Because of the obligation of being made under stress and quickly, IOPC process is an error-prone method every time. Methods: In this study, the IOPC cases that were examined in Dicle University Medical Faculty between the years of 2007-2012 are reviewed. Results: 1758 (97.08%) of 1811 IOPC cases were diagnosed correctly. Of 53 cases that were diagnosed wrongly, 39 (2.15%) and 14 (0.77%) were diagnosed as wrong negative and wrong positive respectively. In our series it is detected that, of 53 wrongly diagnosed cases, the wrong diagnose was related to comment error in 35, to macroscopic sampling error in 15, and to technical error in 3. In the tissues of IOPC cases, the ovary materials constituted the first frequency. The 8 of 449 evaluated ovary cases were diagnosed wrongly and when 27 cases in which the diagnose was left to paraffin sections were excluded, the accuracy rate was determined as 98.11%. Conclusion: In our series, this rate relating to IOPC of ovary tissues seems as the highest accuracy rate in the literature.

Bir üniversite hastanesinde 5 yıllık intraoperatif patoloji konsültasyonlarının değerlendirilmesi

Abstract

Amaç: İntraoperatif patoloji konsültasyonu (İOPK) patolojinin en önemli ve en güç işlemlerinden biridir ve sonuçlarının retrospektif olarak yeniden gözden geçirilmesinin patoloji laboratuarlarında bir kalite kontrol yöntemi olduğu kabul edilmektedir. İOPK işlemi stres altında ve hızlı yapılmak zorunda olduğu için her zaman hataya açık bir yöntemdir. Yöntemler: Bu çalışmada, Dicle Üniversitesi Tıp Fakültesi (DÜTF) Patoloji Anabilim Dalında 2007-2012 yılları arasında incelenen olgular içindeki İOPK'lar değerlendirilmiştir. Bulgular: 1811 İOPK vakasından 1758'ine (%97,08) doğru tanı verilmiştir. Yanlış tanı verilen 53 vakanın 39'una (%2,15) yanlış negatif, 14'üne (%0,77) yanlış pozitif sonuç verilmiştir. Serimizde yanlış sonuç verilen 53 vakadan 35\'inin yorum hatası, 15\'inin makroskopik örnekleme hatası ve 3\'ünün de teknik hata nedeniyle meydana geldiğini tespit ettik. İOPK olgularımıza ait dokular içerisinde birinci sıklığı over materyalleri oluşturmaktadır. İncelediğimiz 449 over dokusundan 8‘ine yanlış sonuç verilmiş ve sonucun parafin takibe bırakıldığı 27 vakayı çıkardığımızda doğruluk oranı %98,11 olarak saptanmıştır. Sonuç: Serimizdeki over dokularının İOPK incelemesine ait bu oran literatürdeki en yüksek doğruluk oranı olarak gözükmektedir.
