Mediastinoskopik Çölomik Kist Eksizyonu

Abstract

Çölomik kistler, nadir görülen konjenital iyi huylu mediastinal lezyonlardır. Genellikle perikardiyal alanda bulunurlar ve altın standart tedavi yöntemi cerrahi olarak çıkarılmalarıdır. Mediastinoskopi ile total eksizyon hakkında literatürde oldukça az sayıda bildiri bulunmaktadır. Bu olguda videomediastinoskopi ile çıkarılan nadir bir çölomik kist sunulmaktadır. Otuz sekiz yaşında erkek hasta, dispne şikâyeti ile başvurdu. Toraks Bilgisayarlı Tomografisi (BT)’nde sağ paratrakeal bölgede saptanan kistik lezyonun Pozitron Emisyon Tomografisi (PET) -BT’sinde Maksimum standardize tutulum değeri (SUVmax) 2.8 saptandı. Hastaya tanısal Videomediastinoskopi planlandı. Genel anestezi altında Videomediastinoskopi ile lezyona ulaşıldı, içeriği aspire edildi ve total eksizyonu gerçekleştirildi. Patoloji incelemesinde kist epitelinde kalretinin ve CK-7 pozitifliği ile çölomik kist tanısı doğrulandı. Postoperatif komplikasyon izlenmedi. Hasta ameliyat sonrası ikinci günde taburcu edildi. Bir aylık kontrol BT’sinde nüks saptanmadı. Videomediastinoskopi, uygun olgularda mediastinal kistlerin tanı ve tedavisinde minimal invaziv, güvenli ve etkili bir yöntemdir.

Mediastinoscopic Excision of a Coelomic Cyst

Abstract

Coelomic cysts are rare congenital benign mediastinal lesions, most commonly located in the pericardial area. Surgical excision is the gold standard treatment. Total excision via videomediastinoscopy has been rarely reported. We present a rare case of a coelomic cyst removed by videomediastinoscopy. A 38-year-old male presented with dyspnea. Thoracic Computed Tomography (CT) revealed a cystic lesion in the right paratracheal area, and Positron Emission Tomography (PET)-CT showed an Maximum standardized uptake value (SUVmax) is 2.8. Diagnostic mediastinoscopy was performed under general anesthesia. Using a videomediastinoscope, the lesion was accessed, its content was aspirated, and total excision was achieved. Histopathological examination confirmed a coelomic cyst with positive calretinin and CK-7 staining. The postoperative period proceeded without complications and the patient was discharged on the second postoperative day. At the 1-month follow-up CT, no recurrence was detected. Videomediastinoscopy can be a safe, effective, and minimally invasive method for the diagnosis and treatment of mediastinal cysts in selected cases.
