Geniş Torakolumbal Yaranın Punch Greft Uygulaması ile Sağaltımı

Abstract

Çalışma olgusunu, dorsalde interskapular bölgeden 3. lumbal vertebraya, distalde bilateral parasternal bölgeye
kadar uzanan deri kayıplı kesik (abrazyon) yarası bulunan iki yaşlı, erkek, Husky ırkı köpek oluşturdu. Klinik muayenede,
purulent akıntı, yara dudaklarında mortifikasyon ve dehidrasyon belirlendi. İki hafta uygulanan medikal sağaltım
ile yara bölgesindeki mortifiye dokular temizlendi. Dördüncü haftada 3 mm çaplı biyopsi punch’ı yardımıyla alınan
sağlam deri, punch greft yöntemi ile yara bölgesine ekildi. Postoperatif ilk hafta günlük pansuman yapıldı. İkinci hafta
iki günde bir pansuman uygulandı. Dördüncü haftada yara bölgesinin tam epitelizasyonu görülerek hasta taburcu
edildi. Geniş yaraların kapatılmasında, punch greft yönteminin sekunder iyileşmeye yardımcı olarak kullanılabileceği
kanısına varıldı.

Large Thoracolumbal Wound Treatment with Punch Graft

Abstract

 A two year old, male, Husky breed, having a cut wound (abrasion) extending from interscapular region
to 3. lumbal vertebrae at dorsal side and to parasternal area bilaterally at distal side constituted the material of this
study. At the clinical examination purulent discharge, mortification of wound margins and dehydration were determined.
Mortified tissues have been cleared after a medical treatment applied to the wound area for two weeks. On the
fourth week, intact skin biopsy punched by a 3 mm diameter tool, planted on to the wounded area with punch graft
method. During the postoperative period, the dressing has been changed every day for the first week, and every
other day in the second and third weeks. On the fourth week full epithelialization of the wounded area was observed
and the patient was discharged. It is concluded that in large wounds, punch graft method can be used to assist secondary
healing process.
