ROBOTİK VE AÇIK RADİKAL PROSTATEKTOMİ YÖNTEMLERİNİN MALİYET ETKİLİLİK ANALİZİ

Abstract

Araştırmada, prostat kanseri tedavisinde kullanılan açık radikal prostatektomi (ARP) ve robotik radikal prostatektomi (RRP) yöntemlerinin maliyet etkililiklerinin değerlendirilmesi amaçlanmış; hizmet başı ödeme kapsamında geri ödeme kurumu perspektifi ve doğrudan hasta maliyeti perspektifinden maliyet ve etkililik verileri hesaplanmıştır. Ayrıca geri ödeme kurumu perspektifine geri ödeme kapsamına alınabilecek (ameliyat için cepten ödeme, ilaç ve muayene) maliyetlerinin eklenmesiyle oluşan üçüncü bir perspektiften de maliyet etkililik analizi gerçekleştirilmiştir. Maliyet verileri Ankara Üniversitesi İbni Sina Hastanesi’nde Mart 2018 – Mayıs 2019 tarihleri arasında 36’sı ARP ve 69’u RRP ile ameliyat olan toplam 105 hasta için belirlenmiştir. Etkililik verileri, EuroQol-Five Dimensional-Five Level (EQ 5D 5L) yaşam kalitesi aracılığıyla elde edilmiştir. Araştırmaya katılan hastalara ameliyattan önce ve ameliyat sonrası üçüncü ay olmak üzere iki kere yaşam kalitesi ölçeği uygulanmıştır. Tüm perspektiflerde RRP daha maliyetli bir yöntem olarak belirlenmiştir. Yöntemler arasında ameliyat sonrası Quality Adjusted Life Year (QALY) farkı robotik radikal prostatektomi lehine 0,07 olarak tespit edilmiştir. Maliyet etkililik analizi sonucunda ilave maliyet etkililik oranları (İMEO), hizmet başı ödeme kapsamında geri ödeme kurumu perspektifinden 9.891,42 TL, doğrudan hasta maliyeti perspektifinden 136.663,14 TL olarak belirlenmiştir. Doğrudan hasta maliyeti içerisinde yer alıp geri ödeme kapsamına alınabilecek maliyetlerin (ameliyat için cepten ödeme, ilaç ve muayene) eklenmesiyle oluşan perspektiften elde edilen İMEO değeri 146.948,14 TL olarak tespit edilmiştir. Belirlenen İMEO değerleri eşik değer ile karşılaştırıldığında RRP, hizmet başı ödeme kapsamında geri ödeme kurumu perspektifinden çok maliyet etkili bir yöntem olarak tespit edilmiştir. Robotik radikal prostatektomi sonrasında daha fazla yaşam kalitesi değeri elde edilse de hastaların tamamı RRP yöntemini tercih edememiştir. Hastaların robotik cerrahi için yapmış oldukları ortalama 9.565,21 TL’lik cepten ödemenin bu durum üzerinde etkili olduğu varsayılmaktadır. Endikasyonu olan hastaların, RRP için cepten ödeme yapmadığı bir geri ödeme politikası ile daha fazla hastanın robotik cerrahi hizmetinden faydalanacağı düşünülmektedir.
