Baş ve boyun defektlerinin onarımında pektoralis major miyokutan flebi kullanımı

Abstract

Baş ve boyun kanserlerinin en etkili tedavi yöntemi olan geniş cerrahi eksizyon sonrası bazen oldukça büyük, kozmetik ve fonksiyonel açıdan sorun oluşturan defektler oluşmaktadır. Bu defektlerin kapatılması, kozmesis sağlanması ve fonksiyonların devam ettirilmesi için farklı rekonstrüksiyon teknikleri kullanılmaktadır. Bu amaçla baş boyun rekonstrüksiyonunda pektoralis major daha sık olmak üzere pediküllü flep kullanımı güncelliğini sürdürmektedir. Mikrovasküler teknikle yapılan serbest pediküllü flepler iyi sonuç vermelerine karşılık özel teknik beceri ve ekipman gerektirmektedir. Pektoralis major miyokutan flebi hareket açısı fazla ve iyi beslenen bir fleptir. Bu çalışmada pektoralis major miyokutan flep uygulanan 3 olgu sunulmuştur. Olguların tümünde defektler başarılı bir şekilde kapatılmış, hiç bir olguda komplikasyon gelişmemiştir

The usage of pectoralis major flap in the reconstruction of head and neck area

Abstract

Large tissue defects may occur as a result of wide resection of tumor in the patients with head and neck carcinoma. The size of tissue defect directly affects post-operative functions and cosmetic results. Several surgical techniques have been applied for reconstruction of the functionality and cosmesis. Even though free tissue transfer is a better way of reconstruction for the majority of most such defects, the use of pedicled pectoral major musculo-cutaneous flap is still in use due to the cost, time, expertise. Pedicled pectoral major musculocutaneous flap technique has relatively fewer complications, the learning curve is short and no specialized training in microvascular surgery is needed. We present three patients treated with pedicled pectoral major musculo-cutaneous flap. All of the cases had succesfull outcome without complication
