KARDIYOMİYOPATİLERIN KLİNİK, EPİDEMİYOLOJİK VE PROGNOSTİK YÖNDEN RETROSPEKTİF OLARAK DEĞERLENDİRİLMESİ

Abstract

Bu çalışmada 84 kardiyomiyopatili hastanın klinik ve epidemiyolojik özellikleri incelenmiş ortalama 10±2 aylık izlem suren sonundaki sonuçları değerlendirilmiş, prognoza etki eden faktörler araştırılmıştır. Çalışmada Primer kardiyomiyopatilerin çoğunun dilate tipte olduğu (%59.2), ardından hipertrofik (%37.28) ve restriktif (%3.38) tipin geldiği, hipertrofik tipte erkeklerin çoğunlukta olduğu; en sık nefes darlığı ve çabuk yorulma ile başvurup hepatomegali ve taşikardi bulgusu ile karşılaşıldığı, ilk başvuruda %66.6 kalp yetmezliği olduğu saptanmıştır.

Hastaların tümünde elektrokardiyograbk, telekardiyografîk ve ekokardiyografik değerlendirmeler yapılmış, gerektiğinde kalp kataterizasyonu uygulanmıştır. EKG’de en sık sol ventrikül hipertrofisi ve ST- T değişikliği ile karşılaşılırken, ilk başvuruda %90.4 haftada telekardiyogramda kardiyomegali saptanmıştır. Hastaların ortalama izlem süresi 10 ±2 ay olup, %10.71 exitus, %34.52 düzelme, %41.66 ayar kalma, %13.09 kötüleşme olduğu görülmüştür. Dilate tipte %5.26, restriktif tipte %33.33, hipertrofik tipte %8.3 oranında serebrovaslriüer olay ile karşılaşılmıştır. Sonuca etki eden faktörlerin istatistiksel analizlerinde cins, yaş, akrabalık, aritmi ve kalp yetmezliği varlığı gibi faktörlerin sonucu istatistiksel olarak anlamlı ölçüde etkilemediği bulunmuştur.
