Fiziksel Tespit Komplikasyon Riski Değerlendirme Ölçeğinin Geliştirilmesi

Abstract

Amaç: Bu çalışma, yoğun bakım hastalarında fiziksel tespit uygulaması sonucunda gelişebilecek komplikasyon riskini değerlendirmek amacıyla yeni bir ölçek geliştirmek için planlanmıştır. Gereç ve Yöntem: Dahiliye yoğun bakım ünitesinde yatan hastalarla yürütülen bu çalışmada geliştirilen ölçeğin madde havuzu, yazarların literatür, fiziksel tespit alanında yürüttüğü komplikasyonları belirleme çalışması ve bu alanda komplikasyon gelişen hasta özellikleri hakkında görüşmelerden de yararlanılarak oluşturulmuştur. Uzman görüşleri ardından hazırlanan form, 88 hastanın 135 ekstremitesinin değerlendirilmesiyle geliştirilmiştir. Verilerin analiz edilmesinde pearson korelasyon analizi, Mann Whitney U testi, ROC analizi, Youden ve Diagnostik indeks, ki-kare ve lojistik regresyon analizi kullanılmıştır. Bulgular: Komplikasyon gelişen ekstremitelerle gelişmeyen ekstremitelerin Fiziksel Tespit Komplikasyon Riski Değerlendirme Ölçek puan ortalamaları karşılaştırıldığında, gruplar arasındaki fark istatistiksel olarak anlamlı bulmuştur (U=890,000, p=0,000). ROC analizi sonucunda, Diagnostik indeks ve Youden indeksi değerlerine göre 4,5 puan kesme noktası olarak belirlenmiş, ölçeğin duyarlılığı 0,72 ve özgüllüğü 0,84 olarak bulunmuştur. Bu sonuca göre, Fiziksel Tespit Komplikasyon Riski Değerlendirme Ölçeğinden 4.5 puan ve üstünde alan hastalar fiziksel tespit sonucunda gelişen komplikasyonlar açısından yüksek riskte olarak değerlendirilmiştir. Ölçeğin üçüncü günde eğri altında kalan alan değeri 0,801(0,719-0,883) olarak bulunmuş olup, bu değerler ölçeğin çok iyi düzeyde ayırıma sahip olduğunu göstermektedir. Ölçeğin, komplikasyon gelişen bireylerin %72’sini yüksek riskli olarak tanımlandığı belirlenmiştir. Sonuç: Çalışmamızın sonuçları, Fiziksel Tespit Komplikasyon Riski Değerlendirme Ölçeğinin, fiziksel tespite bağlı gelişebilecek komplikasyonları tanımlamada etkili bir ölçek olduğunu göstermektedir. Geliştirilen Fiziksel Tespit Komplikasyon Riski Değerlendirme Ölçeğinin yetişkin yoğun bakım ünitelerinde kullanılması önerilmektedir. Development of an Assessment Tool for Physical Restraint Complication Risk

Abstract

Objective: This study was planned to develop a new scale to evaluate the risk of complications that may develop as a result of physical restraint in intensive care patients. Material and Methods: The item pool of the scale developed in this study, which was conducted with patients in the internal medicine intensive care unit, was created by making use of the authors' literature, the study was conducted to determine complications in the field of physical restraint and interviews about the characteristics of patients who developed complications in this field. The form, prepared after expert opinions, was developed by evaluating 135 extremities of 88 patients. Pearson correlation analysis, Mann Whitney U test, ROC analysis, Youden and Diagnostic index, chi-square and logistic regression analysis were used to analyze the data. Results: When the Physical Restraint Complication Risk Assessment Scale score averages of the extremities that developed complications and the extremities that did not develop were compared, found the difference between the groups to be statistically significant (U=890.000, p=0.000). As a result of ROC analysis, 4.5 points were determined as the cut-off point according to the Diagnostic index and Youden index values, the sensitivity of the scale was found to be 0.72 and the specificity was 0.84. According to this result, patients who scored 4.5 points and above on the Physical Restraint Complication Risk Assessment Scale were considered at high risk for complications arising from physical restraint. The area under the curve value of the scale on the third day was found to be 0.801 (0.719-0.883), and these values show that the scale has a very good level of discrimination. It was determined that the scale identified 72% of individuals who developed complications as high risk. Conclusion: The results of our study show that the Physical Restraint Complication Risk Assessment Scale effectively identifies complications that may develop due to physical restraint. The developed Physical Restraint Complication Risk Assessment Scale is recommended for adult intensive care units. Keywords: Physical restraint, nursing, risk, intensive care units.
