Evaluation of the Relationship Between Safe Insulin Administration Knowledge Levels and Metabolic Controls of Adolescents Diagnosed with Type 1 Diabetes

Abstract

Objective: This study aimed to assess the knowledge level of adolescents aged 11–18 diagnosed with Type 1 diabetes regarding safe insulin administration and to examine the possible relationship between this level and metabolic control. Material and Methods: Adolescents aged 11–18 diagnosed with Type 1 diabetes were included in the study. Demographic and clinical information for the adolescents was collected, and their knowledge of insulin administration was assessed using a questionnaire. HbA1c values were used as an indicator of metabolic control. The obtained data were analyzed using Spearman correlation analysis, independent samples t-test, analysis of variance (ANOVA), and Kruskal–Wallis tests. Ethical committee approval, institutional permissions, and informed consent have been obtained for the research. Results: The adolescent’s knowledge level was moderate (mean score: 13.41±2.49). However, no statistically significant relationship was found between knowledge level and demographic or clinical variables, such as age, gender, or duration of diabetes. Group comparisons also revealed no significant difference in knowledge levels. Adolescents’ mean HbA1c level was measured as 8.19±2.1, which was above the ideal glycemic control level. No significant relationship was found between knowledge level and HbA1c. Conclusion: Study results indicate that knowledge of insulin administration alone is insufficient to determine metabolic control. Knowledge level is thought to be influenced by multidimensional factors, including individual learning characteristics, behavioral changes, family support, and motivation. These findings suggest that educational processes should not be limited to the transfer of information.

This research was conducted in accordance with the principles of the Declaration of Helsinki. Ethical approval for the study was obtained from the Health Research Ethics Committee of Izmir Katip Celebi University (Date of Decision: November 14, 2024, Decision No: 0191). Following the approval of the ethics committee, research permission was obtained from the hospital where the research was conducted and from the Bursa Provincial Health Directorate of the Turkish Ministry of Health, to which the hospital is affiliated. Data was collected after obtaining consent from adolescents and their parents via an Informed Consent Form.
