Methemoglobin is a non-functional type of hemoglobin that forms as a result of the oxidation of iron from the ferrous form (+2) to the ferric form (+3). While normal individuals have methemoglobin levels between 1-2%, in patients with methemoglobinemia, tissue perfusion can be severely compromised, with levels of 70% or higher leading to fatal outcomes. In this case,we present the management of a patient refered from emergency department with cyanosis and diagnosed with methemoglobinemia.
Methemoglobin fonksiyonel olmayan bir hemoglobin tipi olup demirin ferröz formundan (+2) ferrik forma (+3) oksidasyonu sonucu oluşur. Normal bireylerde methemoglobin düzeyi %1-2 arası iken methemoglobinemisi olan hastalarda %70 ve üzerinde doku perfüzyonunun bozulmasına bağlı ölümcül seyredebilir. Bu olgu sunumunda acil servise periferik ve santral siyanoz ile başvuran methemoglobinemi saptanan hastanın dahili açıdan yönetimi anlatılacaktır
