Diagnostic Performance of LDL/HDL, CRP/HDL, and Monocyte/HDL Ratios in Predicting Preeclampsia and Severe Preeclampsia

Objective:
Preeclampsia (PE) is a major complication of pregnancy and a leading cause of maternal and fetal morbidity and mortality worldwide. Hyperlipidemia and systemic inflammation are key components of its pathophysiology. This study aimed to evaluate the roles of LDL/HDL, CRP/HDL, and monocyte/HDL ratios in predicting PE and severe PE.
Materials and Methods:
This retrospective single-center cohort study was conducted at Bursa City Hospital between May 2025 and January 2026. A total of 466 pregnant women who delivered between 2021 and 2025 were included. Participants were classified as severe PE (n=72), mild PE (n=156), or healthy controls (n=238). Obstetric outcomes and laboratory parameters, including lipid profile and inflammatory markers, were analyzed.
Results:
LDL levels were significantly higher and HDL levels significantly lower in the PE groups compared with controls (p<0.05). LDL/HDL, CRP/HDL, and monocyte/HDL ratios were significantly elevated both in PE versus controls and in severe versus mild PE (p<0.05). ROC analysis demonstrated that all three ratios had significant diagnostic value in predicting both PE and severe PE.
Conclusion:
LDL/HDL, CRP/HDL, and monocyte/HDL ratios appear to be practical, accessible, and clinically valuable biomarkers for predicting preeclampsia and assessing disease severity.
