Apendikste Low grade müsinöz neoplazm : Tanı ve yönetim

Abstract

Sağ alt kadran ağrısıyla acil servise başvuran 67 yaşında erkek hasta. Akut appendisit appendiks mukoseli ön tanısıyla operasyona alındı. Preoperatif tanı yöntemi olarak kontrastlı batın tomografisi kullanıldı Tomografide apendiks duvari normalden kalin ve uc kisminda ~6x4,5cm lik bir genişliğe ulaşmış ve duvarı proksimalde ilimli diffuz kalin (abse?, mukosel ?) bulundu. Hastaya appendektomi yapıldı Post operatif patoloji sonucu apendiks low grade musinöz neoplazm olarak değerlendirildi. Apendiks tümörleri klinik olarak nadir görülmektedir. İnvaziv bir yapıya sahip olmadığı sürece perop enflamasyonla ayrımı net olarak cerrah tarafından yapılamayabilir. İntraoperatif frozen tanı da kullanılabilir; İntraoperatif frozen kesit değerlendirmesi ile, daha geniş ameliyatların oranını azaltabilir. Operasyonda perforasyon sonucu gelişebilecek psödomiksoma peritonei riskine karşı dikkatli olmamız konusunda bize uyarıcı olacaktır.Kesin tanısı ise patolojik inceleme ile konulmaktadır.
