Retrospective Evaluation of Proton Pump Inhibitors Prescription Data Obtained from Family Medicine Practices in Sakarya

Abstract

Aim: Proton pump inhibitors (PPIs) are prescribed increasingly in the treatment of acid-peptic diseases. PPIs are reported to be one of the most prescribed drug groups, leader in prescription costs, over and long-term prescribed (4-12 months). We aimed to scan the PPIs prescription data of Family Doctors. Methods: In this descriptive study, the number of prescriptions containing PPIs, the distribution of active substances, ICD-10 diagnosis codes, age, gender data of four family doctors in Sakarya were scanned through Family Medicine Data System retrospectively for 3 years (01.08.2014-31.07.2017) after ethical permission. Results: It was determined that there were 15.386 persons registered to four Family doctors, total numbers of examinations are 154.480, total numbers of prescribed drugs are 462.334 in total of 132.081 prescriptions, PPIs are in 16.965 prescriptions with a ratio of 12,84%. Esomeprazole was 34,53%, lansoprazole was 27,93%, pantoprazole was %24,11% of the 6 different active substances. The top three of ICD-10 diagnosis codes were gastroesophageal reflux disease (82,68%), dyspepsia (1,86%), peptic ulcer (1,66%). Females were 61,22% and males were 38,78% of all PPIs prescribed patients and PPIs were prescribed mostly in 41-60 years(38,69%) and 61-80 years(37,73%). Conclusions: We determined that PPIs are in one of eight prescriptions. Attention should be paid to drug interactions and adverse effects in persons over 60 years considering that 43,46% of PPIs are prescribed in this group. The irregular distribution in diagnosis codes may be associated with alterations in repayments of Social Security Institution and habits of doctors. The principles of rational drug use should be applied also for PPIs.

Sakarya’daki Aile Hekimliği Uygulamalarından Elde Edilen Proton Pompa İnhibitörleri Reçete Verilerinin Retrospektif Olarak Değerlendirilmesi

Öz

Amaç: Proton Pompa İnhibitörleri (PPİ), asit-peptik hastalıkların tedavisinde giderek artan oranlarla reçete edilmektedir. PPİ’nin, en sık yazılan ilaç gruplarından olduğu, reçete maliyeti açısından en büyük dilimi oluşturduğu ve hekimlerin bu ilaçları gereğinden fazla ve uzun dönemli tedavi (>4-12 ay) olarak yazdığı bildirilmiştir. Bu araştırmada aile hekimlerinin PPİ reçetelendirme verilerinin taranması amaçlanmıştır. Gereç-Yöntem: Retrospektif tanımlayıcı tipteki araştırmada etik izinler alındıktan sonra Sakarya’da dört Aile Hekiminin, PPİ’Yi içeren reçete sayıları, yazılan etken maddelerinin dağılımları, girilen ICD-10 tanı kodları, cinsiyet ve yaş dağılımlarıyla ilgili veriler Aile Hekimliği Bilgi Sistemlerinden geriye dönük 3 yıllık (01.08.2014-31.07.2017) taranmıştır. Bulgular: Çalışmaya katılan Aile Hekimlerine kayıtlı toplam 15.386 kişinin olduğu, 01.08.2014–31.07.2017 tarihleri arasındaki toplam muayene sayılarının 154.480, yazdıkları toplam ilaç sayısının 462.334 olduğu, bu ilaçların toplam 132.081 adet reçetede yer aldığı, bu reçetelerin 16.965'inde PPİ’nin bulunduğu, PPİ reçetesi oranının %12,84 olduğu belirlendi. PPİ reçetelerindeki 6 farklı etken maddenin %34,53’ü esomeprazol, %27,93’ü lansoprazol, %24,11’i pantoprazol idi ve girilen ICD-10 tanı kodlarında ilk üç sırada gastroözofajiyal reflü hastalığı (%82,68), dispepsi (%1,86), peptik ülser( %1,66) olduğu belirlendi. PPİ reçetelerinin %61,22’sinin kadın, %38,78’inin erkek hastalara ve en fazla 41-60 yaş (%38,69), ve 61-80 yaşları arasında (%37,73) yazıldığı saptandı. Sonuç: Çalışmamızda yaklaşık her 8 reçeteden birinde PPİ’nin yer aldığı bulunmuştur. PPİ’nin %43,46’sı 60 yaş üstü bireylere yazıldığından, bu yaş grubunda ortaya çıkabilecek ilaç etkileşimlerine ve yan etkilere karşı daha ihtiyatlı yaklaşılması gerekmektedir. Tanı kodlarındaki düzensiz dağılımın, Sosyal Güvenlik Kurumu’nun sık aralıklarla değişen geri ödeme uygulamaları ve hekim alışkanlıklarının bir sonucu olabileceği düşünülmektedir. Akılcı ilaç kullanımı ilkelerinin PPİ için de uygulanması gerekmektedir.
