DİYASTOLİK DİSFONKSİYONU OLAN ASEMPTOMATİK HASTALAR İLE SEMPTOMATİK HASTALARIN KARDİYAK SENKRONİZASYON BOZUKLUĞUNUN KARŞILAŞTIRILMASI

Abstract

Amaç: Bu çalışmayla asemptomatik ve semptomatik diyastolik disfonksiyonu olan hastalarda kardiyak asekroninin sıklığını, önemini ve diyastolik kalp yetersizliğinde hastalığa olan katkısını araştırmayı amaçladık.
Gereç ve yöntemler: Asemptomatik diayastolik disfonksiyonu (ADD) olan 50 hasta, semptomatik diyastolik disfonksiyonu (SDD) olan 53 hasta olmak üzere toplam 103 hasta alındı ve bunlara doku doppler ekokardiyografi uygulandı. Sistolik ve diyastolik asenkroniyi belirlemede doku doppler ekokardiyografi ile 6 bazal segment 6 mid segmental bölge kullanılarak toplam 12 segmentte pik sistolik myokardiyal velosite zamanı ( Ts) ve pik erken diyastolik velosite zamanı ( Te ) ölçüldü. Ts ve Te’nin standart deviasyonları ( Ts-SD ve Te-SD ) hesaplandı. Altı bazal segmentte, ortalama sistolik myokardiyal velosite ( Mean Sm) ve ortalama erken diyastolik velosite ( Mean Em ) parametreleri ölçüldü. Sistolik ve diyastolik asenkroninin mevcut olup olmadığı ortaya konulmaya çalışıldı.
Bulgular: Her iki grupta uzamış Ts-SD ( asemptomatik DD grubunda Ts-SD 37,08±15,54 msn, semptomatik DD grubunda Ts-SD 44,50± 19,60 msn; p=0,043) ve uzamış Te-Sd ( asemptomatik DD grubunda Te-Sd 35,53±13,93msn, semptomatik DD grubunda 38,09±16,18 msn;p=0,536 ) gözlendi. Her iki grupta uzamış Ts-SD istatiksel anlamlılığa ulaştı. Asemptomatik DD grubunda Mean Sm 4,47±0,90 cm/sn, semptomatik DD grubunda Mean Sm 4,29±1,11 cm/sn, olup istatiksel olarak anlamlı bulunmadı (p=0,373). Asemptomatik DD grubunda Mean Em 4,74±1,35, semptomatik DD grubunda Mean Em 3,83±1,32, olup istatiksel olarak anlamlı düşük bulundu (p=0,001).
Asemptomatik DD olan hastaların % 48’inde, semptomatik DD olan hastaların %66’sında sistolik asenkroni; asemptomatik DD olan hastaların %54’ünde, semptomatik DD olan hastaların %52,8’inde diyastolik asenkroni mevcuttu.

Sonuç: Hastaların çoğu dar QRS’li EKG özelliklerine sahip olmalarına rağmen hem ADD grubunda hem de SDD grubunda sistolik ve diyastolik asenkroni varlığı gösterildi. SDD grubunda sistolik asenkroni ( Ts-SD) ve erken myokardiyal diyastolik velosite ( mean Em) düşük değeri çalışmamızda istatiksel olarak anlamlı bulundu. Ancak SDD hastalarında diyastolik asenkroni oranı daha fazla değildi.
