Alerjen Spesifik İmmünoterapi: Etkinliği ve Yaşam Kalitesi Üzerine Etkileri

Abstract

Giriş: Alerjen spesifik immünoterapi (AİT) mevsimsel ya da yıl boyu süren alerjik rinit (AR) ve alerjenle tetiklenen astıma sahip olgularda kullanılan bir tedavi yöntemidir. Bu çalışmada, kliniğimizde farklı içerikte standardize alerjen ekstreleri ile konvansiyonel yöntemle subkutan AİT uygulamasının klinik semptomlar, ilaç kullanımı ve yaşam kalitesi üzerine olan etkileri değerlendirilmiştir.
Gereç ve Yöntem: Anket çalışmamızda AİT alan 145 hasta, tedavi süresi ve ekstrelerdeki alerjen içeriklerine göre gruplandırılmıştır. Klinik etkinliği değerlendirme parametreleri olarak, AİT öncesi ve sonrasına ait astım, rinit, konjonktivit semptom skoru ve ilaç kullanım skoru kullanılmıştır. AİT’in etkinliğini hastaların bakış açılarına göre değerlendirmek amacıyla valide edilmiş “Rinokonjonktivit Yaşam Kalite Ölçeği” (RQLQ) ve “Astım Yaşam Kalite Ölçeği” (AQLQ) uygulanmıştır. AİT öncesi ve sonrası alerjik hastalığa ait genel değerlendirme, hasta tarafından subjektif olarak % düzelme ve hekim tarafından visual analog skala (VAS) kullanılarak yapılmıştır.
Bulgular: Çalışmaya katılan 145 hastanın 56‘sı kadın (%38.6), 89‘u erkekti (%61.4). Yaş ortalamasının 36.5±10.3 yıl olduğu görüldü. Tanı özelliğine göre 44 (%30.3) hastanın mevsimsel, 20 (%13.8) hastanın yıl boyu, 81 (%55.9) hastanın ise mevsimsel artış göstermekle birlikte yıl boyu süren yakınmaları vardı. AİT süresi ortalama 3,2 ± 1.25 yıldı. Tedavi etkinliğinin değerlendirilmesinde; AİT öncesine göre AİT sonrası dönemde astım, rinit ve konjonktivit semptom skorlarında ve ilaç kullanım skorlarında azalma gözlendi. Yaşam kalitesi değerlendirmede kullanılan toplam RQLQ ve AQLQ skorunda AİT öncesine göre AİT sonrası belirgin düzelme saptandı. Bu düzelme tüm alt başlıklarda istatistiksel olarak anlamlıydı (p< 0,05).
Sonuç: Subkutan AİT’nin, alerjik rinokonjunktivit ve astım semptomları üzerinde anlamlı klinik etkisinin olduğu ve ilaç kullanımını azalttığı gösterilmiştir. Klinik etkinin hem tek alerjen hem de birden fazla alerjen kullanılarak yapılan uygulamalarda benzer düzeylerde olduğu gözlenmiştir. Klinik etkinliğe paralel olarak tüm yaşam kalitesi ölçeklerinde düzelme saptanmıştır. Yaşam kalitesi ölçümlerinin yapılması erken ve uzun dönemde AİT’in sekonder kazanımlarını değerlendirmek açısından önemlidir.
