Gastrik Liposarkom: Olgu Sunumu

Abstract

AMAÇ: Kliniğimizde takip ve tedavisi yapılan nadir bir gastrik liposarkom olgusunu literatür eşliğinde sunmayı amaçladık. YÖNTEMLER: 63 yaşında kadın hasta, yaklaşık 6 aydır devam eden ve son bir haftadır sıklığı giderek artan epigastrik ağrı ve oral alımda azalma şikayetiyle hastanemize başvurdu. Hasta, kan testleri, özofagogastroduodenoskopi, endoskopik ultrasonografi ve bilgisayarlı tomografi ile değerlendirildi. BULGULAR: Bilgisayarlı tomografide, midenin antropilorik bölgesinde lipomla uyumlu ve lümeni belirgin şekilde daraltan 107x55x65 mm boyutlarında yağlı lezyon görüldü. Özofagogastroduodenoskopide antrumda mide çıkışını daraltan ve postbulber geçişe izin vermeyen lezyondan ve endoskopik ultrasonografide mide antrumunda çevre mukoza ile kaplı büyük submukozal kitleden alınan preoperatif biyopsilerde, hiposelüler ve belirsiz yapıda seyrek dejenerasyon hücreleri ve yağ dokusu parçaları görüldü. Takipte iyi huylu özellikte olan kitlenin progresyonu ve pilor geçişini zorlaştırarak gastrik geçişi tıkaması nedeniyle operasyona karar verildi. Hastaya subtotal gastrektomi + Roux-En-Y anastomoz uygulandı. Histopatolojik inceleme iyi diferansiye liposarkom olarak raporlandı. Ameliyat sonrası birinci gün oral alım başlandı ve hasta ameliyat sonrası üçüncü gün taburcu edildi. Taburculukta endoskopik ve radyolojik takip planlandı. SONUÇ: Gastrik liposarkom, belirgin insidansı olmayan ve atipik semptomları olan nadir bir patolojidir. Morbid komplikasyonları önlemek için erken tanı için daha fazla araştırmaya ihtiyaç vardır. Benzer şikayetleri olan hastalarda, özellikle pilor bölgesinde iyi huylu morfolojiye sahip bir kitleden biyopsi alınması, üst gastrointestinal endoskopi sırasında liposarkomun erken tanısında çok önemlidir. Bu olguda olduğu gibi, her hasta bu kadar şanslı olmayabilir ve daha ölümcül bir sonuçla gelebilir. Gastrik liposarkomun tanısı, tedavisi ve takibi hala belirsiz olduğundan, hastaların değerlendirilmesinde ayırıcı tanıda düşünülmelidir. Güncel literatür verileri ışığında, gastrik liposarkomun günümüzde kabul gören temel tedavisi, daha ileri lenf nodu diseksiyonuna gerek kalmadan temiz sınırlarla cerrahi rezeksiyondur. Son kılavuzlarda iyi diferansiye gastrik liposarkom için adjuvan tedavi önerilmemektedir.

GASTRIC LIPOSARCOMA: A CASE REPORT

Abstract

Liposarcoma represents the most frequently encountered soft tissue sarcoma, comprising approximately 15-20% of all malignant tumors of mesenchymal origin. This neoplasm most frequently arises in the extremities and retroperitoneal space, while visceral involvement is considered uncommon. Gastric liposarcoma is an exceedingly rare condition with limited documentation in the literature. We present a case of well-differentiated gastric liposarcoma in a 63-year-old female patient. The primary aim of this report is to share our clinical experience with this rare entity, while the secondary aim is to evaluate the diagnostic challenges and surgical management strategies based on a literature review. A 63-year-old female patient was admitted to our hospital with complaints of epigastric pain and decreased oral intake persisting for approximately 6 months. Computed tomography revealed a 107 × 55 × 65 mm fat-density lesion compatible with lipoma in the antropyloric region of the stomach, which significantly occupied and narrowed the gastric lumen. Subtotal gastrectomy with Roux-en-Y anastomosis was performed on the patient. Histopathological examination revealed well-differentiated liposarcoma. The postoperative period was uneventful. Postoperative treatment was determined by a multidisciplinary surgical-oncology council, and active surveillance was preferred. Endoscopic and radiological follow-up was planned for the patient after discharge. The patient remained recurrence-free during the 26-month follow-up period. This report highlights the diagnostic challenges of gastric liposarcoma and the need for standardized follow-up.
