Abstract

To evaluate the neuro-ophthalmological examination findings and visual field results in patients with primary headache. Methods: Fifty-five patients diagnosed as primary headache and consulted to Department of Ophthalmology for ophthalmologic examination included in the study. All patients underwent full ophthalmologic examination with neuro-ophthalmologic examination and visual field testing. 110 eyes of 55 patients (45 women, 10 men) were included in the study. The mean age of the patients was 36,56±11,28 (17-66) years. Neuro-ophthalmologic findings associated with headache were detected as ptosis, physiologic anisocoria, limited abduction, optic nerve pallor, peripapillar atrophy, and retina pigment epithelial changes in macula. 14,6% of the eyes had decreased visual acuity. Except for two eyes of one patient, colour vision test was normal in all patients. There was no pathology in pupillary examination. Generalized depression in 68% of eyes and local defects in 51% of eyes were detected in visual field tests. Neuro-ophthalmologic findings were rare in most of the patients with primary headache, but defects and sensitivity losses could be frequently seen in visual fields.

Primer Başağrısı Hastalarında Nörooftalmolojik Bulgular ve Görme Alanı Sonuçları

Abstract

Primer başağrısı olan hastalarda nörooftalmolojik muayene bulgularının ve görme alanı sonuçlarının değerlendirilmesi. Gereç ve Yöntem: Primer başağrısı tanısı alan ve oftalmolojik değerlendirme için Göz Hastalıkları Anabilim Dalı’na konsülte edilen 55 hasta çalışmaya dahil edildi. Tüm hastalar rutin oftalmolojik muayene, nörooftalmolojik muayene ve görme alanı testi ile değerlendirildi. Çalışmaya 55 (45 kadın, 10 erkek) hastanın 110 gözü alındı. Olguların yaş ortalaması 36,56±11,28 (17-66) yıl olarak bulundu. Baş ağrısına eşlik eden göz bulguları pitoz, fizyolojik anizokori, dışa bakış kısıtlılığı, optik sinirde solukluk, peripapiller atrofi, makulada retina pigment epiteli değişiklikleri olarak tespit edildi. Gözlerin %14,6’sında görme keskinliği azalmıştı. Bir hastanın iki gözü dışında tüm olgularda renkli görme testi normaldi. Pupil muayenesinde patoloji yoktu. Görme alanında gözlerin %68’inde yaygın depresyon ve %51’inde lokal hasar saptandı. Primer başağrısı hastalarının nörooftalmolojik muayenelerinde nadiren patoloji saptanırken, bilgisayarlı görme alanı sonuçlarında sıklıkla defektler ve duyarlılık kayıpları görülebilmektedir.
