Abstract

Şilotoraks plevral boşlukta duktus torasikusun bütünlüğünün bozulması sonucu lenfatik sıvının birikmesidir. Nadir görülen bir klinik tablo olmasına rağmen sonuçları itibariyle ve sahip olduğu morbidite ve mortalite potansiyeli nedeniyle benign bir tablo değildir. Etyolojide cerrahi ve cerrahi dışı travma, malignite, enfeksiyon ve diğer çeşitli faktörler yer almaktadır. En sık görülen belirti plevral sıvı birikimi nedeniyle oluşan nefes darlığıdır. Şilöz mayinin plevral yüzeye irritan olmaması nedeniyle göğüs ağrısı ve ateş nadir olarak görülmektedir. Protein, yağ, elektrolit, bikarbonat, lenfosit ve yağda eriyen vitaminden zengin şilöz sıvı kaybı malnutrisyon, güçsüzlük, dehidratasyon, metabolik asidoz ve immünolojik yetmezliğe sebep olmaktadır. Teşhis torasentez ile alınan plevral sıvının biyokimyasal analizi ile doğrulanır. Biyokimyasal olarak trigliserid düzeyi 110mg/dl’den büyük, kolesterol/trigliserid oranı 1’in altında olan efüzyon şilotoraks olarak tanımlanır. Tedavinin ilk basamağını etyolojiyi hızlı bir şekilde belirlemek, ardından buna yönelik konservatif ve/veya cerrahi tedavi planlamasını yapmak oluşturur. Şilöz efüzyon mutlaka drene edilmeli, orta zincirli trigliserid içeren diyet, total parenteral nütrisyon, somatostatin veya octreotid uygulaması, duktus torasikus ligasyonu, plöroperitoneal şant ve plörodez gibi tedavi yaklaşımları, etyoloji ve klinik seyir göz önünde bulundurularak sistematik olarak uygulanmalıdır
