Abstract

This study was conducted to present and discuss our appendectomy series under the light of previous studies. 129 patients admitted with abdominal pain and diagnosed as acute appendicitis between 2008-2010 years were analysed retrospectively. Age, gender, white blood cell count, pathological evaluations of appendectomy specimens, existence of appendecular plug and periappendicular lymph node, intraoperative view of appendix, perioperative ultrasound evaluations and additional diaognosis were analysed. WBC counts were less than 10.000 in 13.2% of the patients, between 10.000-13.000 in 49.6% of the patients and more than 13.000 in 37.2% of the patients. Postoperative pathology classified 85.3% of the patients as appendicitis and 14.7% as normal. Other than 110 patients who were pathologically diagnosed as acute appendicitis, 14 patients had no additional intraabdominal pathology, but 5 of the patiens had additional pathology other than appendicitis during laparotomy process. Altough many perioperative diagnostic methods have been available, negative appendectomy incidance is still remains as a problem. We reached a conclusion that the diagnostic methods should be used either individually or in a combined fashion for prevention of misdiagnosis.

Akut Apandisitte Preoperatif Tetkikler-Laparotomi Korelasyonu

Abstract

Appendektomi tanısında halen kesinlik taşıyan yöntemler bulunmadığı için negatif laparotomilerle çoğunlukla karşılaşılmaktadır. Bu çalışmada kendi appendektomi serimizi güncel literatürler ışığında sunmayı amaçladık. 2008-2010 tarihleri arasında karın ağrısı yakınmasıyla başvuran ve akut apandisit tanısı ile laparotomi yapılmış 129 hasta retrospektif olarak değerlendirildi. Yaş, cinsiyet, beyaz küre sayısı (WBC, /mmolarak), postoperatif appendektomi spesmeninin patolojik inceleme sonucu, appendiküler tıkaç varlığı, periappendiküler lenf nodu varlığı, appendiksin intraoperatif makroskopik görünümü, peroperatif ultrasonografi raporu ve olgulara ilişkin ek tanıları incelendi. Hastalara ilişkin beyaz küre sayıları incelendiğinde 129 adet hastanın %13,2'sinde WBC değeri mm ‘ de 10.000'den az; %49,6'sında mm’de 10.000-13.000 arası; %37,2'sinde mm’de 13.000'den fazladır. Postoperatif patoloji yönünden, 129 adet hastanın %85,3'ünde akut apandisit; %14,7'sinde normal appendiks sonucu görülmüştür. Patoloji sonuçları incelendiğinde 110 olgunun akut apandisit olduğu kabul edilirse; 14 olguda batın içi ek patoloji saptanmamış olup, 5 olguda laparotomi esnasında apandisit dışı ek tanı saptanmıştır. Negatif appendektomiden kaçınmak amacıyla birçok preoperatif yöntem denenmesine rağmen negatif appendektomi insidansinda önemli bir azalma sağlanamamıştır. Uygun olgularda tüm tanı metodlarının tek tek ya da kombine bir halde kullanılarak, muayene tabanlı bir tanıya gidiş sürecinin izlenmesinin önem taşıdığı sonucuna varılmıştır.
