SÜT VE/VEYA YUMURTA ALERJİSİ OLAN ÇOCUKLARDA KIZAMIK-KIZAMIKÇIK-KABAKULAK AŞISININ GÜVENLİĞİ: TEK MERKEZ DENEYİMİ

Abstract

Amaç: Besin alerjisi bulunan çocuklarda bazı aşıların uygulanması konusunda aile hekimleri zaman zaman tereddüt yaşayarak bu hastaları ileri değerlendirme ve uygulama için hastanelere yönlendirmektedir. Bu çalışmada, süt ve/veya yumurta alerjisi tanısı olup Kızamık-Kızamıkçık-Kabakulak (KKK) aşısı için Çocuk Alerji İmmunoloji Polikliniğimize yönlendirilen hastalarda KKK aşısının güvenlik profilini değerlendirmeyi amaçladık.
Gereç ve Yöntemler: Süt ve/veya yumurta alerjisi tanısı alıp Aralık 2023 -Aralık 2024 tarihleri arasında kliniğimizde KKK aşısı yapılan 222 hasta üzerinde retrospektif bir analiz gerçekleştirildi. Hastaların yaş, cinsiyet, eşlik eden alerjik hastalıklar, süt, yumurta sarısı ve beyazı spesifik immünglobulin E düzeyleri, deri prik testleri, aşı ile yapılan prik testi ve aşılama sonrası gelişen reaksiyonlar kaydedildi.
Bulgular: Çalışmaya alınan 222 çocuğun yaş ortalaması 19,3±8,8 ay idi. Hastaların % 56,3’ünde (n:125) yumurta alerjisi, %29,7’sinde (n: 66) süt ve yumurta alerjisi, %14’ünde (n:31) süt alerjisi bulunmaktaydı. Aşı sonrası yalnızca 4 çocukta (%1,8) reaksiyon gelişti; bunlardan biri anafilaksi, üçü ürtiker idi. Reaksiyon gelişen tüm hastaların aşı öncesi aşı ile yapılan prik testi pozitifti.
Sonuç: Bulgularımız, süt ve/veya yumurta alerjisi olan çocuklarda KKK aşısının çoğunlukla güvenle uygulanabileceğini göstermektedir. Ancak, besin alerjisi olan çocuklara aşı yapılırken her aşının bileşimi ve üretim koşullarının dikkatlice incelenmesini ve gerekli durumlarda alerji kliniklerine gönderilmesi gerektiğini düşünüyoruz.

Measles-Mumps-Rubella Vaccine Safety in Children with Milk and/or Egg Allergy: A Single-Center Experience

Abstract

Objective: Primary care physicians may hesitate to administer certain vaccines to children with food allergies and therefore refer them to hospitals for further evaluation and vaccination. In this study, we aimed to evaluate the safety profile of the Measles-Mumps-Rubella (MMR) vaccine in patients diagnosed with milk and/or egg allergy who were referred to our Pediatric Allergy and Immunology Clinic for vaccination.
Material and Methods: A retrospective analysis was conducted on 222 patients diagnosed with milk and/or egg allergy who received the MMR vaccine at our clinic between December 2023 and December 2024. Age, gender, concomitant allergic diseases, specific immunoglobulin E levels for milk, egg yolk, and egg white, skin prick tests, prick testing with the vaccine, and post-vaccination reactions were recorded.
Results: The mean age of the 222 children included in the study was 19.3±8.8 months. Egg allergy was present in 56.3% (n=125), both milk and egg allergy in 29.7% (n=66), and milk allergy in 14% (n=31) of the patients. Post-vaccination reactions occurred in only 4 children (1.8%): one case of anaphylaxis and three cases of urticaria. All patients who developed reactions had a positive prick test performed with the vaccine prior to vaccination.
Conclusion: Our findings indicate that the MMR vaccine can generally be administered safely in children with milk and/or egg allergy. However, we believe that the composition and production conditions of each vaccine should be carefully considered when vaccinating children with food allergies, and that referral to allergy clinics should be made when necessary.
