Konjenital duodenal weblerde radyolojik bulgular

Abstract

Amaç: Konjenital duodenal web tanısı alan olguların klinik, radyolojik bulgularını değerlendirmeyi ve radyolojik yöntemlerin tanıdaki rolünü göstermeyi amaçladık. Yöntem: 2008 – 2022 yılları arasında hastanemizde ösefagus-mide-duodenuma yönelik skopik inceleme (ÖMD) yapılan hastalar retrospektif olarak tarandı ve duodenal web tanısı alan sekiz olgu cinsiyet, yaş, doğum zamanı, eşlik eden konjenital anomaliler, tanı zamanları ve semptomlar yönünden kaydedildi. Radyolojik olarak ayakta direkt batın grafisi (ADBG), abdominal ultrasonografi (USG) ve ÖMD inceleme bulguları değerlendirildi. Bulgular:14 yıllık süreçte duodenal web tanısı alan sekiz olgunun beşi erkek (%62) üçü kız (%38) idi. Yaş aralığı 1 gün - 2 yıl 1 aydı. Bu olguların üçü premature olup gestasyonel yaşları 28-33 hafta arasındaydı. Eşlik eden patolojiler, bir olguda Down sendromu, bir olguda kardiyak anomaliler ve renal agenezi, bir olguda korpus kallosum agenezisi idi. Bir olgu diabetik anne bebeğiydi. Tanı zamanları 5 gün-760 gün arasında değişmekteydi. Olguların ikisinde antenatal USG tanısı mevcuttu. Beş olguda kusma şikayeti mevcut olup birinde kusma safralıydı. Görüntülemede ADBG, abdominal USG ve ÖMD incelemesi yapıldı. ADBG’de üç olguda mide ve duodenal gaz gölgesinde distansiyon, iki olguda ise duodenal gaz gölgesinde distansiyon saptandı. ÖMD incelemesinde olguların hepsinde stenoz proksimalindeki duodenal segmentte dilatasyon mevcuttu. Kontrast maddenin distale geçiş süresi uzamıştı. Beş olguda stenoz bölgesinde radyolüsen hat şeklinde web izlendi. Tüm olgular tetkik sonrası opere edildi. Sonuç: Konjenital duodenal obstruksiyon nedenlerinden biri olan webler gastrointestinal sistemin nadir görülen anomalilerindendir. Webteki açıklığın boyutuna bağlı olarak tanı süresi gecikebilir. Klinik olarak şüphelenilen olgularda duodenal web ayırıcı tanıda düşünülmeli, hastalar skopik olarak ÖMD ile değerlendirilmelidir.

MEU Journal of Health Sciences Assoc was began to the publishing process in 2008 under the supervision of Assoc. Prof. Gönül Aslan, Editor-in-Chief, and affiliated to Mersin University Institute of Health Sciences. In March 2015, Prof. Dr. Caferi Tayyar Şaşmaz undertook the Editor-in Chief position and since then he has been in charge.

Publishing in three issues per year (April - August - December), it is a multisectoral refereed scientific journal. In addition to research articles, scientific articles such as reviews, case reports and letters to the editor are published in the journal. Our journal, which has been published via e-mail since its inception, has been published both online and in print. Following the Participation Agreement signed with TÜBİTAK-ULAKBİM Dergi Park in April 2015, it has started to accept and evaluate online publications.

Mersin University Journal of Health Sciences have been indexed by Turkey Citation Index since November 16, 2011.
