Radyoloji Tanısında Mahremiyet, Sağlık Verisi Paylaşımı ve Yapay Zekâ Uygulamalarına Yönelik Tutumlar: Kesitsel Bir Anket Çalışması

Abstract

Amaç: Bu çalışmanın amacı, radyolojik görüntüleme uygulamalarında hasta mahremiyetine ilişkin algıları, kişisel sağlık verilerinin ikincil kullanımına yönelik tutumları ve yapay zekâ (YZ) ile makine öğrenimi tabanlı uygulamalara duyulan güveni incelemek; mahremiyet algısı ile veri paylaşımı ve YZ’ye güven arasındaki ilişkileri değerlendirmektir. Yöntem: Türkiye genelinde en az bir kez ultrason muayenesi yaptırmış 352 yetişkin bireyin katılımıyla kesitsel, çevrim içi ve anonim bir anket çalışması yürütüldü. Anket; demografik özellikler, ultrason sırasında beden mahremiyeti algısı, sağlık verilerinin ikincil kullanımına yönelik tutumlar ve YZ/makine öğrenimi uygulamalarına ilişkin farkındalık ve güveni içeren dört bölümden oluştu. Mahremiyet algısı 5’li Likert ölçeği ile değerlendirildi. İstatistiksel analizlerde Mann–Whitney U, Kruskal–Wallis ve Spearman korelasyon testleri ile YZ’ye güvenin belirleyicilerini incelemek amacıyla çok değişkenli lojistik regresyon analizi kullanıldı. Bulgular: Ultrason muayenesi sırasında kapı ve perdenin kapalı olması, mahremiyet algısını anlamlı olarak artırdı (p < 0.001). Hekim cinsiyetine bağlı rahatsızlık bildiren katılımcılarda mahremiyet algısı daha düşüktü (p = 0.003). Yüksek mahremiyet algısı, anonimleştirilmiş sağlık verilerinin açık onam olmaksızın kullanımına (p = 0.030) ve ulusal düzeyde oluşturulacak anonim veri havuzlarına desteğin artışıyla ilişkiliydi (p = 0.021). YZ’ye güven kadın cinsiyette daha düşükken (OR = 0.45, p = 0.008), YZ bilgi düzeyi arttıkça anlamlı olarak yükseldi (OR = 1.98, p = 0.004). Mahremiyet algısı, çok değişkenli analizde YZ’ye güvenin bağımsız bir belirleyicisi değildi. Sonuç: Hasta mahremiyeti, fiziksel muayene koşulları ve dijital sağlık teknolojilerine yönelik algılarla birlikte şekillenen çok boyutlu bir kavramdır. Uygun anonimleştirme ve şeffaflık koşullarında, mahremiyet hassasiyeti ile yapay zekâ ve veri paylaşımına yönelik güven algılarının birlikte değerlendirilebileceğini düşündüren bulgular elde edilmiştir.

Privacy in Radiological Diagnosis, Attitudes Toward Health Data Sharing, and Artificial Intelligence Applications: A Cross-Sectional Survey

Abstract

Objective: To assess patients’ privacy perceptions during radiological imaging, attitudes toward secondary use of health data, and trust in artificial intelligence (AI), and to explore relationships between privacy perception, data sharing, and AI trust. Methods: This cross-sectional, anonymous online survey included 352 adults in Türkiye who had undergone at least one ultrasound examination. The questionnaire covered demographics, bodily privacy during ultrasound, attitudes toward secondary data use, and awareness/trust regarding AI and machine learning. Privacy perception was rated on a 5-point Likert scale. Group differences were tested with Mann–Whitney U and Kruskal–Wallis tests; associations with Spearman correlation; and independent predictors of AI trust with multivariable logistic regression. Results: Privacy perception was higher when doors and curtains were closed (p < 0.001). Discomfort related to physician gender was associated with lower privacy perception (p = 0.003). Higher privacy perception was associated with greater acceptance of access to anonymized data without explicit consent (p = 0.030) and support for a Ministry of Health–managed anonymized repository (p = 0.021), although effect sizes were small. AI trust was lower in females (OR = 0.45, 95% CI: 0.24–0.81; p = 0.008) and increased with higher self-reported AI knowledge (OR = 1.98, 95% CI: 1.24–3.16; p = 0.004). Privacy perception was not an independent predictor of AI trust. Conclusion: Privacy in radiology is multidimensional, and the findings suggest that concerns about privacy may be compatible with support for secure, anonymized secondary data use under transparent ethical governance.
