Abstract

ÖZET:
Antikoagülan tedavi, klinik açıdan geniş bir spektrum oluşturan tromboembolik
hastalıkların profilaksi ve tedavisinde yaşamsal önem taşıyan, ileri kardiyak
cerrahi operasyonlar sonrası kullanımı zorunlu olan bir tedavi yöntemidir. Bu
tedavinin tromboz veya kanamaya yol açmayacak şekilde planlanması, klinik ve
laboratuvar takibinin titizlikle yapılması oldukça önem taşımaktadır. Oral
antikoagülanlara bağlı hemobilia (safra kesesi içine kanama) çok nadir
görülmektedir. Hemobilia; cerrahi travmayı takiben, karaciğer biyopsisi
sonrası, anevrizmalar, extra veya intrahepatik safra yolu tümörleri, safra
taşları ve karaciğerin özellikle parazitik ve pyojenik inflamatuar lezyonları
nedeniyle oluşabilen safra yollarına doğru olan kanamadır. Bu sunumda aort
kapak replasmanı (AVR) ve atrial fibrilasyon (AF) nedeniyle profilaktik olarak
warfarin alımına bağlı spontan safra kesesi kanaması gelişen bir olgu
sunulmuştur.

RARE CASE; ANTICOAGULANT THERAPY INDUCED GALLBLADDER HEMORRHAGE-JAUNDICE

ABSTRACT:
Anticoagulant therapy is a vital method of treatment and prophylaxis of
thromboembolic diseases which clinically have a wide spectrum, and essential
after cardiac valve surgery. Therefore, it is vital that anticoagulant therapy be
planned so that it does not lead to thrombosis or bleeding and clinical and
laboratory follow-up should be performed carefully. Gallbladder (GB) hemorrhage
(hemobilia) is a rare occurence during standard anticoagulation therapy.
Hemobilia is a hemorrhage  into the
biliary tract that may follow surgical trauma, liver biopsy, aneurysms,
extra-intrahepatic tumors of the biliary tract, gallstones, and inflammatory
lesions of liver, especially helmintic or pyogenic. In this  article, a case, who developed hemobilia
caused by requiring warfarin as prophylaxis because of aortic valve replacement
(AVR) and atrial fibrillation, was presented.
