Evaluation of Uncinate Process Upper Attachment Sites and Ethmoid Roof Variations in Pediatric Population

Abstract

Objective: The aim of the study is to evaluate the uncinate process upper attachment site (SAUP) and depth of the olfactory fossa (OFD) variations in the pediatric population with the help of computed tomography (CT), to examine a relationship between them and to create awareness in order to avoid complications in endoscopic sinus surgery. Materials and Methods: A total of 420 people who met the appropriate criteria were included in the study, and 840 side reviews were conducted. CT images taken in the coronal plane were examined digitally in the bone window, and OFD measurements were made according to the Keros classification. In addition, Landsberg's and Friedman's classification was used in the evaluation of the SAUP and the results were compared with statistical methods. Distributions were compared across age groups, and the association between SAUP and OFD/Keros categories was analyzed. Results: According to the Keros Classification, 23.7% of the patients were Keros I, 67.1% were Keros II, and 9.2% were Keros III. When the distribution of the SAUP was examined, there was no statistically significant correlation between the OFD and the SAUP. Conclusion: In this pediatric cohort, OFD demonstrated clear age-related variation, whereas SAUP patterns showed no significant association with OFD/Keros categories. These findings support age-conscious interpretation of preoperative pediatric CT imaging, particularly when planning frontal recess–related procedures.
