Stomalı Hastaların Yaşadıkları Sorunlar Sorun Çözme Becerileri ve Depresyon Arasındaki ilişki

Abstract

Giriş ve Amaç: Ostomi açılması bireylerin yaşam süresini uzatabilir ve sağlıklı bir hayata yeniden dönmelerine yardım edebilir. Stomalı bireyler bu süreçte fizyolojik, sosyal ve psikolojik sorunlar yaşayabilir. Beden imajında değişiklik, düşük özgüven, yalnızlık, umutsuzluk ve damgalanma, sosyal izolasyon ve cinsel sorunlar gibi. Bu sorunlar nedeniyle hastanın ruh sağlığı olumsuz etkilenebilir ve depresyon gibi ruhsal sorunlara neden olabilir. Bu çalışma, stomalı bireylerin yaşadıkları sorunlar, sorun çözme becerileri ve depresyon arasındaki ilişkiyi belirlemek amacıyla yapılmıştır.

Gereç ve Yöntem: Tanımlayıcı nitelikte olan bu araştırma, 15 Haziran 2018- 15 Şubat 2019 tarihleri arasında bir üniversite hastanesinin stomaterapi ünitesi’nde gerçekleştirilmiştir. Araştırmanın örneklemini 89 hasta oluşturmuştur. Verilerin toplanmasında Beck Depresyon Ölçeği (BDÖ) ve Sosyal Sorun Çözme Envanteri (SSÇE) kullanılmıştır.

Bulgular: Katılımcıların ortalama yaşı 51.73±14.95, katılımcıların %53.9’u erkektir. Katılımcıların %91.0’inin stomayla ilgili sorun yaşadığı, katılımcıların BDÖ puan ortalamasının 38.43±10.07 ve SSÇE toplam puanının 64.21±12.45 olduğu belirlendi. BDÖ ve SSÇE arasında negatif yönde ve anlamlı bir ilişki bulundu.

Sonuç: Bu çalışmada stomalı hastaların, stomayla ilgili pek çok alanda sorun yaşadıkları, sorun yaşayan hastaların depresyon düzeylerinin daha yüksek olduğu, sorun çözme becerileri ile depresyon arasında negatif yönde anlamlı bir ilişki olduğu belirlenmiştir. Bu nedenle stoma hemşiresi ve konsültasyon liyezon psikiyatri (KLP) hemşiresinin hastaların psikososyal sorunlarını çözebilmek için, stomalı hastanın bakımında birlikte çalışmaları önerilmiştir.

The relationship between the problems experienced by stoma patients problem- solving skills and depression

Abstract

Introduction and Purpose: An ostomy could prolong individuals’ life and help them have a healthy life again. In this process, individuals with a stoma can experience physiological, social, and psychological problems. Some examples include changes in body image, low self-confidence, loneliness, hopelessness, stigmatization, social isolation, and sexual problems. The patient’s mental health could be affected negatively by these problems, which could lead to psychological problems like depression. This study aims to identify the relationship between the problems experienced by stoma patients, problem-solving skills, and depression.
Material and Method: This descriptive study was conducted at the stomatheraphy unit of a university hospital between the 15th of June 2018 and the 15th of February 2019. The sample was composed of 89 patients. Data were collected through the Beck Depression Scale (BDS) and the Social Problem-Solving Inventory (SPSI).
Results: The average age of the participants was 51.73 ±14.95 and 53.9% of them were males. Of all the participating individuals, 91.0% reported to have experienced problems with the stoma, the participants’ average BDS score was 38.43±10.07, and the SPSI total score was 64.21±12.45. A negative and significant relationship was found between the BDS and SPSI.
Conclusion: This study found that stoma patients experienced many problems related to the stoma, and the depression levels of the patients who experienced problems were higher, and there was a negative and significant relationship between problem-solving skills and depression. Therefore, stoma nurses and Consultation-Liaison Psychiatry (CLP) nurses are recommended to collaborate in the care of the stoma patients to solve their psychosocial problems.
