Palyatif bakım alan hasta yakınlarının bakıma hazır olma düzeyleri ve bakım yüklerinin belirlenmesi

Abstract

Giriş ve Amaç: Palyatif bakım alan hastalara sıklıkla hasta yakınları tarafından bakım desteği verilmektedir. Bu durum hasta yakınlarında bakım vermeye hazırlıklı olma ve bakım vermenin oluşturduğu yükler ortaya çıkarmaktadır. Bu çalışma palyatif bakım alan hasta yakınlarının bakıma hazır olma düzeyleri ve bakım yüklerinin belirlenmesi amacıyla tanımlayıcı olarak yapılmıştır.
Gereç ve Yöntem: Araştırma bir Üniversite Hastanesinin palyatif bakım kliniğinde yatan hastaların yakınları ile yürütülmüştür. Araştırma örneklemini 180 hasta yakını oluşturmuş ve çalışma için etik kurul ve kurum izni alınmıştır. Araştırma verileri Edmonton Semptom Değerlendirme Sistemi, Bakım verme yükü ve Bakıma Hazıroluşluluk ölçeği kullanılarak yüz yüze toplanmıştır. Araştırmanın değerlendirilmesinde Kruskal Wallis ve Pearson Korelasyon testleri kullanılmıştır.
Bulgular: Hasta yakınlarının % 56,1’ inin kadın, %44,4’ ünün ( 31-40 ) yaş aralığında, %62,2’ sinin evli olduğu tespit edilmiştir. Ailelerin Bakım verme yükü ölçeği alt boyut puan ortalaması 44,97±12,33 ve bakım vermeye hazır oluşluluk ölçeği alt boyut puan ortalaması 28,96±6,28 olduğu tespit edilmiştir. Hasta yakınlarının bakım yükü ile bakıma hazır oluşlulukları arasında negatif bir ilişki tespit edilmiştir (p<0,05). Hasta yakınlarının bakım yükü ile hastalarının yaşadığı semptom toplam puanı arasında pozitif yönde bir ilişki bulunmaktadır. (p<0,05).
Sonuç: Hasta yakınlarının bakıma hazıroluşlulukları arttıkça bakım yükünün azaldığı saptanmıştır. Hastanın yaşadığı semptom yoğunluğu arttıkça hasta yakınlarının bakım yükünün de arttığı saptanmıştır.

Determınatıon of care preparedness levels and care burden relatives of patient receiving palliative care

Abstract

Introduction and Aim: Patients receiving palliative care are often given care support by the patient's relatives. This situation causes the burden of care in the relatives of the patients and the unpreparedness to provide care. This is study determination of care preparedness levels and care burden relatives of patients receiving palliative care for descriptive purposes.
Materials and Methods: The research was conducted with relatives of patients hospitalized in the palliative care clinic of a University Hospital. The research sample consisted of 180 patient relatives and ethics committee and institutional permission was obtained for the study. Research data were collected face to face using the Edmonton Symptom Assessment System, Caregiving Burden and preparedness for caregiving scale. Kruskal Wallis and Pearson Correlation tests were used in the evaluation of the research.
Results: 56.1% of the patient's relatives are female, 44.4% (31-40) years old, 62.2% are married. It was determined that the average score of the families in the Caregiving Burden and Caregiving Preparedness Scale was 44.97±12.33 and 28.96±6.28. As a result, a negative relationship was found between caregiver burden and preparedness to care (p<0.05). There is a positive correlation between the care burden of the relatives of the patients and the total score of the symptoms experienced by the patients (p<0.05).
Conclusion: It has been determined that as the patient's relatives' preparedness for care increases, the care burden decreases. It has been determined that as the intensity of symptoms experienced by the patient increases, the care burden of the patient's relatives also increases.
