ALLERJİK ÇOCUK OLGULARDA ÖZGÜL İMMUNOGLOBULİN E DÜZEYLERİ İLE ALLERJENLERİN BELİRLENMESİ VE TOTAL IgE, IgA, IgM VE IgG DEĞERLERİ

Abstract

Elazığ ve yöresinde alerjik reaksiyonlara neden olan allerjenlerin belirlenmesi amacıyla, Elazığ Devlet Hastanesi ve Fırat Üniversitesi Fırat Tıp Merkezi'nin çocuk polikliniklerine allerjik şikayetle başvuran ve alerji ön tanısı alan, yaşları 2-12 arasında değişen, İmmünoloji laboratuvarında total IgE, IgA, IgM, IgG ve özgül IgE düzeyleri belirlenen 77 (yaş: 5.92 ± 0.33) çocuk olgu çalışma kapsamına alındı. Olgulara, ayrıntılı bir anket uygulanmış ve fiziksel muayeneleri gerçekleştirilerek ön tanıları belirlenmiştir.
37 vakada sadece allerjik astım, 30 vakada ise sadece ürtiker tablosu gözlenirken, 6 olguda ürtiker + allerjik astım, 2 olguda allerjik rhinitis + allerjik konjonktivit, 1 olguda allerjik astım + allerjik dermatit ve 1 vakada da ürtiker + allerjik dermatit birlikte tespit edilmiştir. Allerjik olgular ve aynı yaş grubundaki sağlıklı bireyler (n=30 yaş: 5.53 ± 0.26) arasında IgG, IgA, IgM değerleri (mg/dL) nefelometrik tekniklerle ölçümlendiğinde istatistiki anlamlı bir farklılık gözlenmemiştir. Total IgE düzeyleri (kIU/L) ise allerjik olgularda (201.41 ± 19.55) , kontrol grubuna (39.835 ± 7.27) kıyasla yüksek bulunmuştur (p<0.05).
Olguların multialerjen panellere vermiş olduğu IgE yanıtı ölçümlenmiş ve olguların pozitif yanıt verdiği panellere göre dağılımı sırasıyla gıda paneli (Fx4, n:27), akarlar ve evcil hayvan tüylerini içeren ev tozları paneli (HMx1, n:20), polen paneli (Bx5, n:14), hayvan deri ekleri (Ex1, n:10), fungus paneli (Mx6, n:10) olarak tespit edilmiştir. Alerjenlere spesifik IgE tespiti yapıldığında en sık rastlanan spesifik alerjenler yumurta beyazı (f1, n:35), kedi epiteli (e1, n:29), inek sütü (f2, n:25), köpek tüyü (e2, n:19) ve akarlar (d1 ve d4, n:29) olarak bulunmuştur (International Allergen Code:IAC). Sonuç olarak multialerjen panellere vermiş olduğu IgE yanıtı ölçümlenmiş ve olguların pozitif yanıt verdiği panellere göre en sık pozitif yanıt görülen panelin gıda paneli (Fx4), Alerjenlere spesifik IgE tespiti yapıldığında ise en sık rastlanan spesifik alerjenin yumurta beyazı (f1) olduğu görülmüştür.
Total IgG, IgA, IgM seviyeleri Beckman Array®360 Systems cihazıyla, aynı firmanın ayıraçları kullanılarak (Beckman, Kaliforniya, U.S.A.), nefelometrik yöntemle mg/dL cinsinden ölçülmüştür. Total IgE ve özgül IgE düzeyleri ise serumda ELİSA yöntemi ile üretici firmaların tarifi üzerine ölçümlenmiştir (Total IgE; Int. Immuno-Diagnostics, USA, Özgül IgE; Dr. Fooke, Neuss, Germany).

Abstract

Purpose: In our study, we aimed to determine allergens cause allergic reactions in the region of Elaz!#. Methods: The total IgE, IgA, IgM, IgG and specific IgE levels of 77 children are represented whose ages between 2 and 12 (5.92±0.33) years and had attended with allergic complaints to the pediatric clinics of Elazig Community Hospital and Firat Medical Center of Firat University. We determined 37 were only allergic asthma, 30 were only urticaria; in 6 cases urticaria and allergic asthma were together, in 2 cases allergic rhinitis and allergic conjunctivitis were together, in one case allergic asthma and allergic dermatitis and in an other one case urticaria and allergic dermatitis were together. The total IgE and specific IgE levels in serum was measured by ELISA. Results: When we compared the IgG, IgA and IgM values (mg/dL, which are determined with the nephelometric technique) of the allergic cases with the same aged healthy ones (n:30; ages 5.53±0.26), there was no statistically significant diversity. The total IgE values (kIU/L) of the allergic cases (201.41±19.55) were statistically significantly higher than the control group (39.835±7.27) (p<0.05). Conclusions: The IgE response of the cases to the multi-allergen panel were measured and the distribution of the cases that had positive results to the panels were respectively: food panel (Fx4, n:27), house dusts panel, consists of mites and pets$ fur (HMx1, n:20), pollen panel (Bx5, n:14), animal skin addiction (Ex1, n:10), fungus panel (Mx6, n:10). The most common specific IgE to the allergens that are determined were; egg white (f1, n:35), cat epithelium (e1, n:29), caw milk (f2, n:25), dog feather (e2, n:19) and mites (d1 and d4, n:29) (International Allergen Code:IAC). The total IgG, IgA and IgM levels were assessed by the device of Beckman Array®360 Systems.
