Maternal Vitamin D Düzeyi Kötü Obstetrik Prognozla İlişkili Mi?

Abstract

Amaç: Birinci trimester 25 hidroksi vitamin D seviyesi ve kötü obstetrik prognoz arasındaki ilişkiyi belirlemeyi amaçladık. 
Yöntem: Birinci trimesterde 11-14 hafta taraması için başvuran 180 tekil gebelik olgusundan ardışık olarak kan örneği alındı. Tüm gebeler doğuma kadar takip edildi ve 144 hasta çalışmayı tamamladı. Tüm hastaların 11-14 haftada birinci ve 22-24 haftada ikinci trimester ultrasonografi ve Doppler incelemeleri yapıldı. Serum 25 hidroksi vitamin D düzeyi 11-14 hafta arasında ölçüldü. Fetal ölüm, preeklampsi, gebeliğin tetiklediği hipertansiyon, gebelik yaşına göre düşük doğum ağırlıklı bebek (SGA) doğumu, preterm doğum ve gestasyonel diyabet gelişimi kötü obstetrik prognoz olarak kabul edildi. 
Bulgular: Kötü obstetrik prognoz gelişen gebeliklerdeki 25 hidroksi vitamin D düzeyi normal prognoz ile sonuçlanan gebeliklerdekinden daha düşüktü, fakat bu sonuçlar istatistiksel olarak anlamlı bulunmadı (p<0,05).. 
Sonuç: Vitamin D yetersizliği taramasının, gebelik öncesi dönemde yapılmasının daha uygun olacağını düşünmekteyiz. Gebelik istemi olan kadınlar, vitamin D yetersizliğinin yol açtığı perinatal komplikasyonlar için bilgilendirilmeli ve gebelik öncesi dönemde vitamin D desteği uygulanmalıdır.
