The Effect of Medical Home Model on the Developmental Outcomes at 12-18 Months Corrected Age of Very Low Birth Weight Infants Born and Followed up in a Tertiary Hospital

Abstract

The aim of this study is to examine the effect of the medical home model (MHM) on the developmental status of very low birth weight (VLBW) infants at 12- 18 months corrected age in a tertiary health center in Turkey. Material- Method: A total of 199 infants who were born with birth weight ≤ 1500 grams and/or gestational week ≤ 32 weeks at Zekai Tahir Burak (ZTB) Maternity Teaching Hospital Ankara and received medical and developmental follow-up at ZTB hospital within the medical home model were enrolled. Their developmental status was evaluated at corrected 12- 18 months with Bayley II. Results: Of the study group, 49% was female and 51% was male. Mean birth weight was 1238 ± 319 grams and 25,1% (n=50) was ≤ 1000 grams. Mean gestational age was 29, 2±2,2 weeks and 38,2% was ≤ 28 weeks. About 50% was multiplets. Bayley Mental Development Index scores (MDI) and Psychomotor Development Index scores (PDI) were < 70 in 7% and 19,6% of the total group, respectively and 7% was diagnosed as cerebral palsy (CP). Bayley MDI and PDI scores were < 70 in 12% and 32% of the infants born ≤ 1000 grams and 12% had CP. Conclusion: The low rate of babies with unfavorable developmental status in the study group suggests that MHM may help the early recognition and intervention of major developmental problems in high risk babies and decreases later problems. Long term studies are needed to determine the effect of MHM on minor developmental problems of the VLBW infants. Key words: Preterm; Health Care Delivery; Medical Home; Children.

Üçüncü Basamak Sağlık Hizmeti Veren Bir Hastanede Taburculuk Sonrasında Bütüncül İzlem Modeli İle İzlenen Çok Düşük Doğum Ağırlıklı Bebeklerin Düzeltilmiş 12-18 Ayda Gelişimsel Değerlendirmesi

Öz

Giriş: Prematüre ve/veya çok düşük doğum ağırlıklı doğan bebeklerde sağlık ve gelişimsel sorunların görülmesi açısından yüksek riskli bebeklerdir. Yüksek riskli bu bebeklerin izleminin tüm koruyucu ve tedavi edici hizmetlerin aynı ekip tarafından, aynı “çatı altında” verilmesini sağlayan “Bütüncül İzlem modeli (BİM)” ile yapılması önerilmektedir. Amaç: Bu çalışmada 1 yıllık dönemde T.C.Sağlık Bakanlığı Dr. Zekai Tahir Burak Kadın Sağlığı Eğitim ve Araştırma Hastanesinde doğan, yenidoğan kliniğinde yatan ve taburculuk sonrasında aynı merkezin Gelişimsel Pediatri ünitesinde izlenen çok düşük doğum ağırlıklı bebeklerin düzeltilmiş 12-18 aylarındaki gelişimsel sonuçlarının belirlenmesi amaçlandı. Materyal - Metod: Doğum ağırlığı ≤ 1500 gram ve/veya gestasyonel haftası ≤ 32 hafta olan, taburcu edilen ve izlemleri tamamlanmış olan toplam 199 bebek çalışmaya alındı. Bebekler taburculuk sonrasında BİM kapsamında, belirli aralıklarla aynı gelişimsel pediatri ünitesinde bir gelişimsel pediatrist ve bir çocuk gelişimcisi tarafından değerlendirildi ve uygun gelişimsel destekler verildi. Düzeltilmiş 12-18 aylarında bebeklerin nörolojik muayeneleri ve “Bayley Bebekler için Gelişimsel Değerlendirme Ölçeği II” ile gelişimsel değerlendirmeleri yapıldı. Bulgular: Çalışma grubunda yer alan 199 bebeğin %49'u kız ve %51'i erkekti. Bebeklerin ortalama doğum ağırlığı 1238 ±319 gram ve %25,1'inin (n=50) doğum ağırlığı ≤ 1000 gram idi. Bebeklerin ortalama doğum haftası 29,2±2,2 hafta ve %38,2'sinin ≤ 28 hafta idi. Bebeklerin yaklaşık %50'si çoğul gebelikti. Yapılan gelişimsel değerlendirmede bebeklerin %7'sinin bilişsel ölçek puanı < 70, %19,6'sının hareket ölçek puanı < 70 olarak saptandı. Bebeklerin %7'si serebral palsi tanısı aldı. Doğum ağırlığı 1000 gram ve altında olan toplam 50 bebeğin %12'sinin bilişsel ölçek puanı < 70 ve %32'sinin hareket ölçek puanı < 70 olarak belirlendi. Bebeklerin %12,0'si serebral palsi tanısı aldı. Sonuç: Çalışma grubumuzdaki bebeklerimizin 12-18 aydaki gelişimsel sonuçlarının iyi olmasının başarılı yenidoğan yoğun bakım koşulları, BİM kapsamında iyi bir izlem, yeterli gelişimsel destek ve sorunların erken fark edilip yönlendirme yapılmasına bağlı olabileceği düşünülmektedir.
