Multi-Planar Orientation of the Talar Neck in Dry Adult Tali: An Osteological Study with Reference Values for Fracture Reduction

Abstract

The talar neck has an oblique three-dimensional configuration that may influence the interpretation of talar morphology and the assessment of talar neck fractures. Although several studies have described selected aspects of talar geometry, osteological reference data evaluating transverse, sagittal, and frontal plane orientation together remain limited. This study aimed to measure the multiplanar orientation of the talar neck in dry adult tali using standardized digital photographs. Eighty-seven adult tali, including 51 left and 36 right specimens, were assessed; after exclusions, 83 specimens were included in the final analysis. Standardized transverse, sagittal, and frontal photographs were obtained for each talus, and angular measurements were performed digitally using ImageJ. Three parameters were defined: Talar Neck Anteversion Angle in the transverse plane, Talar Neck Declination Angle in the sagittal plane, and Varus–Valgus Angle in the frontal plane. Right tali were digitally mirrored to a left-foot coordinate system. The mean values were 162.38° ± 7.89° for anteversion, 164.73° ± 6.91° for declination, and 141.77° ± 8.57° for varus–valgus orientation. Anteversion was higher on the left, whereas varus–valgus orientation was lower on the left. Declination did not differ between sides. These findings provide reference values for talar neck orientation in three anatomical planes and suggest that talar neck morphology should not be interpreted from a single plane alone.
