Portal Hypertension Associated With Splenic Artery Aneurysm

Abstract

The splenic artery is the third most common site of intraabdominal aneurysms after aneurysms of the abdominal aorta and the iliac arteries. Aneurysms of the splenic artery are a rare complication of portal hypertension in adults because of increased splenic artery blood flow as a result of the hyperdynamic circulation that occurs in cirrhosis. They are more common in females and also occur in association with diabetes mellitus, vascular disease such as arteriosclerosis, multiparity and alpha-1-antitrypsin deficiency. Most true splenic artery aneurysms are asymptomatic and are diagnosed incidentally. The risk of rupture increases with pregnancy, with portal hypertension, and after liver transplantation. We present a case of two aneurysms of distal segmental branch of splenic artery in adults with chronic liver disease and portal hypertension.

Portal Hipertansiyonla Birlikte Splenik Arter Anevrizması

Abstract

Splenik arter, abdominal aorta ve iliak arterlerden sonra üçüncü en sık intraabdominal anevrizma yeridir. Splenik arter anevrizmaları, sirozda, hiperdinamik sirkülasyon sonucu, splenik arter kan akımının artmasıyla oluşan portal hipertansiyonun nadir bir komplikasyonudur. Kadınlarda sıktır ve diabetes mellitus, ateroskleroz gibi vasküler hastalıklar, multiparite ve alfa-1 antitripsin eksiliğinde de görülebilir. Birçok gerçek splenik arter anevrizması asemptomatiktir ve tanı rastlantısal olarak konur. Rüptür riski gebelik, portal hipertansiyon ve karaciğer transplantasyonu sonrası artar. Amacımız, splenik arter disal segmental dalında iki anevrizması olan, kronik karaciğer hastalığı ve portal hipertansiyonlu olguyu sunmaktır.
