Lateral Malleoplasty For Traumatic Lateral Malleol Instability Of The Ankle: A Case Report

Abstract

The lateral malleolus is essential for the lateral stabilisation of the ankle. Defective lateral stabilisation may arise from a congenital absence of the fibula or traumatic loss of the lateral malleolus. There are several surgical techniques to resolve this problem. Within published literature, a new technique arousing a great deal of interest is that of triangular bicortical autograft taken from the iliac wing together with a lateral malleoplasty. We present here the functional results of the case of an 8-year-old boy, who was inside the vehicle in a traffic accident, and suffered a type 3B open fracture with a loss of 10 x 7cm soft tissue in the anterolateral left ankle, bone tissue loss of the distal fibula and anterolateral distal tibia . Our aim was to restore the lateral stability of the ankle using the malleoplasty technique.

Lateral Malleol Travmatik Kaybı Sonucu Gelişen Ayak Bileği Lateral İnstabilitesinde Lateral Malleolplasti

Abstract

Lateral malleol ayak bileğinin önemli bir lateral stabilizatörüdür. Doğuştan fibula yokluğu veya travmatik olarak lateral malleolun kaybı, ayak bileği lateral stabilitesinin bozulmasına neden olmaktadır. Bu sorunu gidermek için çeşitli cerrahi yöntemler uygulanmaktadır. Literatür incelendiğinde yeni bir teknik olan, iliak kanattan alınan triangüler bikortikal otogreft ile lateral malleolplasti uygulaması oldukça ilgi uyandırmıştır. Biz araç içi trafik kazası sonucu sol ayak bileği anterolateralinde 10×7 cm ebadında yumuşak doku kaybı ile beraber, fibulanın distali ve tibia anterolateralinde kemik doku kaybı ile giden, Gustillo Anderson tip 3B açık kırıklı 8 yaşındaki erkek hastanın, ayak bileği lateral stabilitesini sağlamak amacıyla lateral malleolplasti tekniğini kullanarak elde ettiğimiz fonksiyonel sonucu sunmayı amaçladık.
