LAD perforation of a patient during percutaneous coronary intervention (PCI).

Abstract

Coronary perforation is an uncommon (%1-2<), but serious complication of pci. The causes of coronary perforation include rupture of the vessel wall due to oversized balloons; deep vessel wall trauma from the use of directional atherectomy, rotational atherectomy, laser angioplasty, balloon inflation in a false lumen during recanalization of chronic total occlusions and wire perforation in a small branch vessel.The severity of the perforation determines its clinical course. Coronary perforations are classified as class I-II and III. We report a patient with ladmperforation by using an oversize balloon during pci, the hemodynamic parameters progresively worsed and treated with emergency cabg.

Perkütan Koroner Girişim Sırasında Sol Ön İnen Arter (LAD) Perforasyonu Gelişen Bir Olgu

Abstract

Perkütan koroner girişimler sırasında gelişebilen perforasyon son derece nadir (%1-2<) fakat ciddi bir komplikasyondur. Over-size balon kullanımı, direksiyonel aterektomi, rotasyonel aterektomi ve lazer anjiyoplastiye bağlı derin duvar hasarı, kronik total oklüzyonlarda yalancı lümende balon şişirilmesi ve küçük yan dallarda kılavuz tel kırılması en önemli koroner perforasyon nedenleridir. Perforasyonun büyüklüğü kliniğin ciddiyetini belirler. Koroner perforasyonlar klas I-II-III olmak üzere üç gruba ayrılırlar. Biz burada sol ön inen artere perkütan girişim sırasında over-size balon kullanımına bağlı perforasyon gelişen, hemodinamisi şiddetle bozulan ve acil cerrahi ile tedavi ettiğimiz bir hastayı sunduk
