Treatment Planning and Dosymmetric Comparison of Two Different Volumetric Arc Therapy Technique

Abstract

The purpose of this study is to compare and evaluate the performance of two different volumetric modulated arc therapy techniques. In this study, 20 patients with late stage nasopharyngeal cancer were selected for comparison of the plans. All treatment plans were selected to have 3 target volumes, doses of 70/60/54 Gy, and were generated from 33 fractions with simultaneous integrated boost technique. All treatment plans were made with Elekta VMAT using the Monaco TPS and RapidArc using Eclipse TPS. Plans were compared in terms of the 95% volume of target goal, OAR, CI, HI, and MU. Significant differences were observed in all criteria. In the RapidArc, the results were slightly lower (6913,65 ± 64,80 / 5972,02 ± 89,88 / 5391,10 ± 155,83) while the Elekta VMAT yielded better results in terms of target volume doses (6813,65 ± 79, 48 / 5926,36 ± 99,34 / 5347,65 ± 69,89). CI in Elekta VMAT (0,997±0,010 / 0,989±0,020 / 0,997±0,006) and HI (0,070±0,028 / 0,210±0,022 / 0,175±0,42) is slightly better than CI (0,991±0,011 / 0,949±0,000 / 0,950±0,001) and HI (0,075±0,016 / 0,255±0,018 / 0,210±0,54) in RapidArc. The MU of RapidArc (516,59 ± 68,03) was significantly lower than the Elekta VMAT MU (1213,30 ± 131,90).

İki Farklı Volumetrik Ayarlı Ark Terapi Tekniğinin Tedavi Planlaması Ve Dozimetrik Karşılaştırılması

Abstract

Çalışmamızda iki farklı volumetrik ayarlı ark terapi tekniğinin performansını karşılaştırmak ve değerlendirmek amaçlandı. Bu çalışmada planların karşılaştırılması için 20 adet geç evre nazofarenks kanser tanılı hasta seçildi. Tüm tedavi planları 3 hedef hacim, dozları 70/60/54 Gy olacak şekilde seçildi ve 33 fraksiyondan eşzamanlı entegre boost tekniği ile oluşturuldu. Tüm tedavi planları Monaco® tedavi planlama sistemini kullanan Elekta VMAT ve Eclipse TPS kullanan RapidArc ile yapıldı. Planlar hedefin %95lik hacimin dozu, riskli organların dozu, konformite indeksi, homojenite indeksi ve Monitor değeri açısından karşılaştırıldı. Tüm kriterlerde anlamlı farklılıklar gözlendi. Elekta VMAT tekniğinde hedef hacim dozları bakımından daha iyi sonuçlar elde edilirken (6919,65±64,80 / 5972,02±89,88 / 5391,10±155,83) RapidArc tekniğinde sonuçlar biraz daha düşük bulundu (6813,65±79,48 / 5926,36±99,34 / 5347,65±69,89). Elekta VMAT tekniği çoğu riskli organ dozunda RapidArc tekniğine daha düşük sonuçlar verdi. Elekta VMAT tekniğinde hedef hacim içindeki konformite (0,997±0,010 / 0,989±0,020 / 0,997±0,006) ve homojenite indeksi (0,070±0,028 / 0,210±0,022 / 0,175±0,42) bakımından RapidArc ın konformite (0,991±0,011 / 0,949±0,000 / 0,950±0,001) ve homojenite indeksinden (0,075±0,016 / 0,255±0,018 / 0,210±0,54) daha iyi sonuçlar verdiği gözlendi. RapidArcın monitor değeri (516,59±68,03) Elekta VMAT monitor değerinden (1213,30±131,90) belirgin bir farklı daha düşük olduğu gözlendi.
