HER2 pozitif meme karsinomu dokularında CD1a ekspresyonu ve prognostik önemi

Öz

Giriş:
Dendritik hücreler, birincil bağışıklık yanıtının başlatılmasında temel rol oynayan hücrelerdir. Meme karsinomunda antijen sunan hücrelerin rolünü araştıran çok sayıda çalışma, dendritik hücre yoğunluğu, tümör içindeki dağılımları ve prognoz arasında çeşitli ilişkiler ortaya koymuştur. CD1a, insan tümörlerinde dendritik hücre belirteci olarak yaygın şekilde kullanılan glikozile bir tip I transmembran proteindir. Bu çalışmada, HER2 aşırı ekspresyonu gösteren meme karsinomu hastalarında CD1a ekspresyonunun immünohistokimyasal yöntemlerle tespiti ve bu ekspresyonun prognostik parametreler ve genel sağkalım ile ilişkisi incelenmiştir.

Yöntem:
Bu retrospektif çalışmada, 2003 ile 2009 yılları arasında invaziv meme karsinomu tanısı almış hastaların kayıtları, xxx Fakültesi Patoloji Anabilim Dalı dijital patoloji arşivinden elde edilmiştir. Bu veri setinden, HER2 pozitifliği doğrulanmış 48 olgu belirlenen dahil etme kriterlerine göre seçilmiştir. Tüm olgulara CD1a immünohistokimyasal boyama uygulanmış ve boyama sonuçları prognostik verilerle karşılaştırılmıştır.

HER2 negatif olan, ya da 2+ skorlanıp SISH ile pozitifliği doğrulanmayan olgular; immünohistokimyasal çalışma için uygun olmayan hücre bloklarına sahip olgular ve prognostik verilerine ulaşılamayan olgular çalışma dışında bırakılmıştır.

Bulgular:
CD1a ekspresyonu ile sağkalım ve prognostik faktörler arasındaki ilişki değerlendirilirken her düzeydeki CD1a pozitifliği dikkate alınmıştır. CD1a pozitifliği ile yaş, tümör boyutu, lenf nodu tutulumu veya nüks arasında istatistiksel olarak anlamlı bir ilişki bulunmamıştır. CD1a ekspresyonu ile tümörün neoadjuvan tedavi durumu arasında da bir ilişki saptanmamıştır. Ancak, nükleer derece ile CD1a pozitifliği arasında anlamlı bir ilişki bulunmuştur. CD1a boyanması ile 5 yıllık toplam sağkalım ve hastalıksız sağkalım arasında ise istatistiksel olarak anlamlı bir ilişki tespit edilmemiştir.

Sonuç:
HER2 pozitif tümörlerin meme karsinomlarının yalnızca %25–30’unu oluşturduğu düşünüldüğünde, bu çalışma belirli ve homojen bir hasta grubuna odaklanmıştır. HER2 pozitifliğinin kötü prognozla ilişkili olması ve çalışmaya erken evre dışı olguların dahil edilmiş olması nedeniyle daha düşük sağkalım oranı beklenmiştir. HER2 pozitif olgularda CD1a pozitifliği ile sağkalım arasında orantılı bir ilişki gözlenmiş olsa da, bu ilişki istatistiksel olarak anlamlı bulunmamıştır. Daha kesin sonuçlara ulaşılabilmesi için gelecekteki çalışmalarda daha geniş hasta serileri veya prognostik özellikler açısından daha heterojen gruplar değerlendirilmelidir. Bu çalışmada CD1a pozitifliği yüksek nükleer derece ile anlamlı şekilde ilişkili bulunmuştur.

CD1a expressionin Her2-positive breast carcinoma tissues and its prognostic implications

Öz

Aims: Dendritic cells are key players in initiating the primary immune response. Many studies have investigated the role of antigen-presenting cells in breast carcinoma, revealing associations between dendritic cell density, their spatial distribution within the tumor, and prognosis. CD1a is a glycosylated type I transmembrane protein that is frequently used as a dendritic cell marker in human tumors. This study aims to detect CD1a expression using immunohistochemical methods in a cohort of HER2-overexpressing breast carcinoma patients. It also analyzes the correlation of CD1a expression with prognostic parameters and overall survival outcomes.
Methods: Patient records of individuals diagnosed with invasive breast carcinoma between 2003 and 2009 were retrieved from the digital pathology archive of İstanbul University İstanbul Faculty of Medicine Pathology Department. From this dataset, 48 cases were selected based on the following inclusion criteria: HER2 positivity confirmed. We applied cd1a immunohistochemically to all of these cases and compared the staining with prognostic data.
Results: Any level of CD1a positivity was considered when evaluating the association between CD1a expression and survival/ prognostic factors. No statistically significant relationship was found between CD1a positivity and age, tumor size, lymph node involvement, or recurrence. No correlation was identified between CD1a positivity/negativity and the neoadjuvant status of the tumour. However, a significant association was observed between nuclear grade and CD1a positivity. No statistical correlation was found between 5-year total and disease-free survival and CD1a staining.
Conclusion: Considering that HER2-positive tumors account for only 25–30% of breast carcinomas, our study focused on a particular and homogeneous patient group. A lower survival rate was expected because HER2 positivity is a known poor prognostic factor, and our cohort consisted of non-early-stage cases. Therefore, while a proportional relationship between CD1a positivity and survival was observed in HER2-positive cases, statistical significance was not achieved. Future studies should include larger patient populations or more heterogeneous cohorts in terms of prognostic features to obtain more conclusive results. In our study, CD1a positivity was significantly associated only with higher nuclear grade.

The study protocol was approved by İstanbul Üniversitesi İstanbul Tıp Fakültesi Clinical Research Ethics Committee granted the ethical approval for the study (approval number: 2012/1504-1231, date: 04/01/2013). Informed consent forms were not obtained from the patients as the study was conducted retrospectively.
