DIAGNOSTIC VALUES OF TRANSTHORACIC FINE-NEEDLE ASPIRATION AND TRANSTHORACIC TRU-CUT BIOPSY IN NON-SMALL-CELL LUNG CANCER

Objective: In this study, we aimed to evaluate the clinical feasibility of computed tomography (CT)-guided percutaneous transthoracic needle biopsy (PTNB) by analyzing its sensitivity, specificity, complication rates, and diagnostic utility in the histopathological characterization of thoracic malignancies. Methods: Data on 864 patients who underwent transthoracic biopsy were analysed retrospectively. After excluding non-operated patients and those with insufficient data, a total of 279 procedures performed on 202 patients were analyzed. Age, gender, smoking status, comorbidity, lesion location, and dimension, needle entry distance, procedure duration, pneumothorax-hemothorax status, and pre- and postoperative pathology parameters were analysed. Results: When the results of Tru-cut biopsy, fine-needle aspiration biopsy (FNAB), and the groups where both were performed together were compared with post-operative pathology results; the sensitivity and specificity were calculated separately for lung adenocarcinoma, squamous cell lung cancer, lung carcinoid tumor, undiagnosed benign cases, and large cell lung cancer. No significant difference was observed between the groups (p=0.807). Pneumothorax and pulmonary hemorrhage were the most frequently encountered complications. Pneumothorax occurred in 29.2% of patients, while pulmonary hemorrhage was detected in 73.8%. Conclusion: Transthoracic biopsy remains a valid diagnostic tool today, with its high diagnostic rate, minimal invasiveness, short recovery time, and low complication rates. It will maintain its importance in lung cancer diagnosis until histopathology is obtained with less invasive methods.
