OUTCOMES OF IMAGE-GUIDED DRAINAGE FOR POSTOPERATIVE ABDOMINOPELVIC COLLECTIONS

Öz

Objective: Image-guided percutaneous drainage is widely used as a first-line treatment for postoperative intra-abdominal and pelvic collections; however, determinants of re-intervention remain incompletely defined. Methods: This single-center, retrospective observational study included patients who underwent ultrasound- and/or CT-guided percutaneous drainage for intra-abdominal or pelvic collections after colorectal surgery between January 2023 and December 2025. Hospital information system and PACS records were reviewed, and only the index drainage procedure per patient was analyzed. Demographic variables, imaging findings (maximum diameter, septations, intracavitary gas), procedural factors (imaging guidance, access route, catheter size, catheter dwell time), and microbiology results were recorded. The primary endpoint was re-intervention within 30 days (repeat percutaneous drainage and/or surgery for the same collection). Group comparisons and multivariable logistic regression were performed. Results: A total of 101 patients were included (median age 64.0 years). Catheter sizes were 8F (n=37), 10F (n=49), and ≥12F (n=15). Re-intervention within 30 days occurred in 31/101 patients (30.7%) and did not differ significantly across catheter-size groups (p=0.222). Catheter dwell time was longer in patients who required re-intervention (median 25.0 vs 18.5 days; p=0.045). In multivariable analysis, catheter size was not independently associated with re-intervention; intracavitary gas and catheter dwell time showed non-significant trends. Thirty-day mortality was 1.0%. Conclusion: In postoperative colorectal surgery–related intra-abdominal/pelvic collections, catheter size was not independently associated with 30-day re-intervention in this retrospective cohort. However, this finding should be interpreted cautiously because catheter size was not randomly assigned. Catheter dwell time and intracavitary gas may be clinically relevant and warrant evaluation in larger prospective cohorts.
