Adli otopsilerde kalp ağırlığının değerlendirilmesi

Öz

AMAÇ: Çalışmada kalp ağırlığının Adli Tıp pratiğinde daha iyi değerlendirilmesini sağlamak amaçlandı. YÖNTEM: Daha önceden bir hastalık hikayesi olmayan, 18-35 yaş arası erkek olguların otopsilerinde alınan kalpler bütün olarak tartıldıktan sonra keskin diseksiyonla epikardiyal yumuşak doku, sol ventrikül serbest duvarı, sağ ventrikül serbest duvarı ve septum bölümlerinin ağırlıkları ayrı ayrı kaydedildi. Yaşa, boya ve vücut kitle endeksine göre toplam kalp ağırlığı ile ventriküler ağırlıkların dağılımları tespit edildi. SPSS programı v20 kullanılarak bu değerlerin her iki cinsiyette ayrı ayrı olmak üzere “vücut ağırlığı”, “boy” ve “vücut kitle endeksi” değişkenleriyle korelasyon gösterip göstermediği araştırıldı. Korelasyon var ise ve anlamlı bir regresyon modeli oluşturulabiliyorsa tekli ve çoklu regresyon denklemleri oluşturuldu. Korelasyon katsayıları ve regresyon denklemlerinin kestirim güçleri literatürdeki başlıca postmortem kalp ağırlığı çalışmaları ile karşılaştırıldı. BULGULAR: Olgularda vücut ağırlığı-toplam kalp ağırlığı arasında yüksek r=0,680 düzeyde; vücut ağırlığı-yağsız toplam kalp ağırlığı arasında orta r=0,573 düzeyde; vücut ağırlığı-sol ventrikül ağırlığı arasında düşük r=0,336 düzeyde; boy-toplam kalp ağırlığı arasında orta r=0,461 düzeyde; boy-yağsız toplam kalp ağırlığı arasında yüksek r=0,614 düzeyde; boy- sol ventrikül ağırlığı arasında yüksek r=0,604 düzeyde; vücut kitle endeksi-toplam kalp ağırlığı arasında orta r=0,539 düzeyde; vücut kitle endeksi- yağsız toplam kalp ağırlığı arasında zayıf r=0,347 düzeyde; vücut kitle endeksi- sol ventrikül ağırlığı arasında düşük r=0,359 düzeyde korelasyon saptandı. SONUÇ: Yapılan çoklu regresyon analizlerinde TKA = {-1203.297 + [ -5.510 x VA] + [ 23.252 x VKE] + [ 7.876 x Boy]} denklemi kuruldu. Bu denklemin kestirim değeri R2 %52,6 olarak hesaplandı.

Evaluation of heart weights in forensic autopsies

Öz

INTRODUCTION: The aim of this study was to provide better information for the evaluation of heart weights in forensic medical practice. METHODS: In the autopsies of cases with no history of disease, the hearts were weighed as a whole and after sharp dissection, the weights of epicardial soft tissue, the left ventricular free wall, the right ventricular free wall, and septum sections were recorded separately. Total heart weight TKA and the distributions of ventricular weights were determined according to age, height, and body mass index. SPSS software v20 was used to determine whether TKA, heart weight without epicardial soft tissue YTKA , right ventricular weight SaVA , septal weight StA , and left ventricular weight SoVA values were correlated with “body weight VA ,” “height Boy ,” and “body mass index VKE .” Single and multiple regression equations were established if there was a correlation, and a significant regression model could be formed. The correlation coefficients and regression equations’ predictive validities were compared with the primary postmortem heart weight studies in the literature. RESULTS: There was a high-level correlation r = 0.680 between VA and TKA, a moderate-level correlation r = 0.573 between VA and YTKA, a low-level correlation r = 0.333 between VA and SoVA, and a moderate-level correlation r = 0.461 between Boy and TKA. Additionally, there was a high-level correlation r = 0.614 between Boy and YTKA, a high-level correlation r = 0.604 between Boy and SoVA, and a moderate-level correlation r = 0.539 between VKE and TKA. Finally, there was a low-level correlation r = 0.347 between VKE and YTKA and a low-level correlation r = 0.359 between VKE and SoVA. CONCLUSION: In multiple regression analyses, the equation TKA = {-1203.297 + [ -5.510 x VA] + [ 23.252 x VKE] + [ 7.876 x Boy]} was formulated. The R2 value was calculated as 52.6%.
