Alkol veya İlaçla Kolaylaştırma Şüphesi Olan Cinsel Suç Olgularının Retrospektif Değerlendirilmesi

Öz

Amaç: Bu çalışma, alkol veya ilaçla kolaylaştırıldığı şüphesi bulunan cinsel saldırı olgularını değerlendirmek, bu olgularda izlenmesi gereken adli ve tıbbi süreci ortaya koymak amacıyla gerçekleştirilmiştir.
Yöntem: 01 Haziran 2014 - 31 Aralık 2023 tarihleri arasında Muğla Eğitim Araştırma Hastanesi Adli Tıp Polikliniğine cinsel saldırı şikayetiyle başvuran 339 olgu retrospektif olarak incelenmiş, bu olguların 32’sinde alkol veya ilaç kullanımıyla kolaylaştırılmış cinsel saldırı şüphesi belirlenmiştir. Olgulara ait demografik veriler, saldırgan-mağdur ilişkisi, olay yeri ve zamanı gibi değişkenler analiz edilmiştir. Veriler, frekans ve yüzde dağılımlarıyla değerlendirilmiştir.
Bulgular: Tüm olgular kadınlardan oluşmaktadır. Yaş aralığı 14–49 olup, %78.1’inde saldırganla mağdur arasında bir güven ilişkisi tespit edilmiştir. Olayların %59.4’ü ev ortamında gerçekleşmiş, %65.6’sında yalnızca alkol, %12.5’inde alkolle birlikte başka maddelerin kullanıldığı bildirilmiştir. Mağdurların %75’i olay anını kısmen veya tamamen hatırlayamamış, %46.9’u halsizlik, güçsüzlük gibi belirtiler yaşamıştır. Fiziksel travma bulguları genellikle erken başvurularda saptanırken, ruhsal belirtiler daha geç başvurularda ön plana çıkmıştır.
Sonuç: Bu çalışmada, cinsel suçların değerlendirilmesinde mağdurların ifadelerinin dikkatle alınmasının ve olayın hemen ardından biyolojik örneklerinin toplanmasının önemi vurgulanmıştır. Başvuru süresi geciktikçe fiziksel delillerin toplanmasının zorlaştığı, ancak ruhsal etkilerin daha belirgin hale geldiği gösterilmiştir. Bu nedenle, tüm olgular bütüncül bir şekilde değerlendirilerek, geç başvuru durumlarında dahi örneklerin alınması ve mağdurun ruhsal durumu açısından takip edilmesi gerektiği sonucuna varılmıştır

Retrospective Evaluati̇on of Sexual Assault Cases Wi̇th Suspected Alcohol/Drug-Faci̇li̇tation

Öz

Objective: This study aimed to evaluate sexual assault cases with suspected alcohol or drug facilitation and to outline the forensic and medical procedures required in such cases.
Methods: A retrospective analysis was conducted on 339 sexual assault cases reported at Muğla Training and Research Hospital’s Forensic Medicine Outpatient Clinic between June 1, 2014, and December 31, 2023. Among these, 32 cases involved suspected alcohol or drug-facilitated sexual assault. Demographic data, perpetrator-victim relationships, location/time of the incident, and other variables were analyzed. Data were assessed using frequency and percentage distributions.
Results: All victims were female, aged 14–49 years. A trust-based relationship between the perpetrator and victim was identified in %78.1 of cases. Incidents occurred in home environments in %59.4 of cases. Alcohol alone was involved in %65.6 of cases, while %12.5 involved alcohol combined with other substances. %75 of victims reported partial or complete memory loss during the assault, and %46.9 experienced symptoms such as weakness or fatigue. Physical trauma findings were more common in early presentations, whereas psychological symptoms predominated in delayed reports.
Conclusion: This study highlights the importance of meticulously documenting victim statements and collecting biological samples immediately after the incident. As the delay in reporting increases, obtaining physical evidence becomes more challenging, but psychological impacts become more pronounced. Therefore, a holistic approach is essential—even in delayed presentations, samples should be collected, and victims must receive psychological follow-up.
