Extraction of impacted third molars is a very common procedure with numerous risks of complications. These complications are usually seen when the tooth is in close relation with the inferior alveolar nerve IAN and the lingual cortex. Success of the operation and/ or operating without harming adjacent structures depends on the radiographical evaluation and meticulus preoperative examination of the patient. The aim of this case report is to emphasize the importance of careful preoperative clinical and radiological examination to minimize pre and post operative complications such as damage to IAN. Another aim of the report istavascular structures and adjacent dental tissues during surgical removal of a third molar in close relationship with a complex odontoma and the IAN but also give information about a patient in which the IAN is located between the root of impacted third molar in close relation with relatively large complex odontoma
