Oral Cerrahide Koagulasyon Tetkikleri: Neden ve Ne Zaman?

Öz

Oral ve maksillofasiyal cerrahlardan çok geniş bir aralıktaki medikal problemli hastaları tedavi etmesi beklenmektedir. Koagülasyon bozukluğu olan hastalar en zor vaka grupları arasında yer alırlar. En zararsız prosedürlerde dahi intra- veya post-operatif kanama hayatı tehdit edici komplikasyonlara yol açabilmektedir. Platelet sayımı, kanama zamanı, PT, aPTT cerrahi pratikte elektif cerrahiden önce kanama riski için hastaların incelenmesini sağlayan koagülasyon değerlendirilmesinin vazgeçilmez parçaları olarak yer edinmişlerdir. Koagülasyon tetkikleri klinik açıdan yüksek kanama riski grubunda olmayan hastalarda cerraha kanama riskini önceden haber vermez. Özel koagülasyon testleri hemostatik bir anomali olduğundan şüphe edilen kanama hikayesi olan hastalarda endikedir

Öz

Oral and maxillofacial surgeons are expected to treat patients with a wide spectrum of medical maladies. Patients with disorders of coagulation are among the most challenging patients to treat. Intra- or postoperative bleeding can contribute to lifethreatening complications in even the most “benign” surgical procedures. Platelet count, bleeding time, PT and aPTT have become widely ingrained in surgical practice as components of a coagulation panel to screen patients for bleeding risk before elective surgery. Coagulation tests do not predict surgical bleeding risk in patients who are not found to be increased risk on clinical grounds. Specialized tests of coagulation are indicated in patients whose bleeding histories are suspicious for a hemostatic abnormality
