Menstrüel Siklus İlişkili Herediter Pankreatit: Nadir Bir Olgu Sunumu

Öz

Özet Kalıtsal pankreatit, tekrarlayan akut pankreatit ataklarına neden olan nadir bir genetik bozukluktur. Kalıtsal pankreatit olgularının bir alt grubu menstrüel siklus ile ilişkilidir. Bu olgularda, pankreatit atakları hemen hemen her zaman menstrüel siklusun ilk iki günü içinde ortaya çıkar. Menstrüel siklus ile ilişkili kalıtsal pankreatit vakaları nadirdir. Bu olgu, literatürde bildirilen dördüncü vaka olması nedeniyle kayda değerdir. 35 yaşında kadın hasta, tekrarlayan pankreatit atakları nedeniyle başvurdu. Hasta, son bir yıl içinde 5 kez pankreatit atağı geçirmişti. Tüm pankreatit atakları, menstrüel siklusun birinci veya ikinci gününde başlamıştı. Hastanın genetik analizinde, CFTR geninde heterozigot bir mutasyon tespit edildi. Literatür verileri ışığında hastaya 0.03 mg etinil estradiol ve 3 mg drospirenon içeren oral kontraseptif tedavisi başlandı ve takibe alındı. Sekiz aylık takip süresinde hiçbir tekrarlayan pankreatit atağı gözlenmedi. Tekrarlayan pankreatit atakları olan kadın hastalarda, olası bir menstrüel siklus ilişkisi özellikle araştırılmalı ve gerekli genetik analizler yapılmalıdır.

MENSTRUAL CYCLE-ASSOCIATED HEREDITARY PANCREATITIS: A RARE CASE REPORT

Öz

Abstract A subset of hereditary pancreatitis cases are associated with the menstrual cycle and pancreatitis attacks occur almost always within the first two days of the menstrual cycle. Hereditary pancreatitis cases associated with the menstrual cycle are rare. This case is notable as it represents the fourth case reported in the literature. A 35-year-old female patient presented with recurrent pancreatitis attack had experienced 5 episodes of pancreatitis within the last year. All pancreatitis attacks began on the first or second day of her menstrual cycle. Genetic analysis of the patient revealed a heterozygous mutation in the CFTR gene. In light of the literature data, the patient was started on an oral contraceptive containing 0.03 mg ethinyl estradiol and 3 mg drospirenone and was followed up. No recurrent pancreatitis attacks were observed during the eight-month follow-up period. In female patients with recurrent pancreatitis attacks, a possible association with the menstrual cycle should be specifically investigated.
