Is fallopian tube epithelium responsible for the development of ovarian serous carcinoma?

Öz

ABSTRACT

Purpose: Ovarian surface epithelium (OSE) is blamed for ovarian
tumor pathogenesis. Recent publications also blamed tubal epithelium which
exfoliated on ovarian surface (exfoliation theory) for high grade ovarian
carcinogenesis. To assess the value of exfoliation theory, we examined serous
adenoma, borderline serous tumor and serous carcinoma cases.

 

Materials and Methods: Archives of the Department of
Antalya Training and Research Hospital was used to retrieve all biopsy reports
containing the terms” serous cystadenoma (CA), borderline serous tumor (BST)
and serous carcinoma (SC)” and slides were evaluated histopathologically.

 

Results: Among the CA, BST and SC cases papillomatosis was seen in
%5,2-%37,5-%11 respectively; nuclear stratification was seen in %10,5-%25-%11
respectively; nuclear pleomorfism was seen in %79-%50-%33 respectively;
invagination was seen in %26-%50-%33 respectively and inclusion was seen in
%79-%62,5-%33 respectively. Tubal changes of these cases are as follows:
Papillomatozis %70-%14-%25 respectively; nuclear stratification %100-%100-%75
respectively and nuclear pleomorfism %100-%100-%91,6 respectively.

 















Conclusion: Owing to gradual increase on
nuclear stratification, invagination, and papillomatosis between serous
cystadenoma and borderline serous tumor sequence, it seems that ovarian surface
epithelium may play an important role on ovarian serous tumorigenesis. Due to
low frequency of same changes observed in carcinoma, there must be at least two
different pathways among adenoma-borderline lesion and carcinoma pathogenesis.

Seröz over kanseri gelişiminden tuba epiteli mi sorumludur?

Öz

ÖZ

Amaç: Over tümör patogenezinde over yüzey epiteli (OYE)
suçlanmaktadır. Son yıllarda tubadan over yüzeyine dökülen hücreler
(eksfoliasyon teorisi) özellikle yüksek dereceli over karsinogenezinde
suçlanmaktadır. Eksfoliasyon teorisinin değerini saptamak için seröz adenom, borderline
seröz tümör ve seröz karsinom vakalarımızı inceledik.

 

Araçlar ve Yöntem: Antalya Eğitim ve Araştırma Hastanesi
Patoloji Bölümü arşivi seröz kistadenom (KA), borderline seröz tümör (BST) ve
seröz karsinom (SK) kelimeleri kullanılarak tarandı ve histopatolojik bulgular
değerlendirildi.

 

Bulgular: İncelenen KA, BST ve SK vakalarında
OYE’de papillomatozis sırasıyla %5,2-%37,5-%11; nükleer stratifikasyon
%10,5-%25-%11;  nükleer pleomorfizm
%79-%50-33; yarıklanma %26-%50-%33 ve inklüzyonlar %79-%62,5-%33 oranında
görüldü. Her üç grupta tuba epitelinde izlenen değişiklikler ise şöyledir:
KA-BST-SK'da sırasıyla papillomatozis %70-%14-%25; nükleer stratifikasyon
%100-%100-%75 ve nükleer pleomorfizm %100-%100-%91,6.

 















Sonuç: Seröz kistadenom ve borderline seröz
tümör gelişiminde over yüzey epitelinde nükleer stratifikasyon, yarıklanma ve
papillomatozis dereceli bir artış varlığı görülüyorken karsinomda benzer bir
eğilimin olmaması, adenom-borderline lezyon ve karsinom gelişiminde farklı 2
yolağın bulunduğunu göstermektedir.
