Hepatoid adenocarcinoma with liver metastasis: A case report

Hepatoid adenocarcinoma is a special type of extra-hepatic adenocarcinoma, and has a special morphologic similarity to hepatocellular carcinoma. The stomach is one of the most common localizations of hepatoid adenocarcinomas. A 47-year-old man was referred to our clinic due to detection of the large solid masses in the liver. A very high level of serum alpha-fetoprotein was determined, but the ultrasonographic examination of the abdomen showed noncirrhotic liver parenchyma and the viral serologic markers were negative. Percutaneous liver biopsy was performed. The pathological examination of the biopsy material revealed metastatic hepatoid type adenocarcinoma. Upper gastrointestinal endoscopy showed gastric ulcers in the antrum. Histological examination of the antral biopsy specimens revealed malignant epithelial tumor.

Karaciğere hepatoid adenokarsinoma metastazı: Olgu sunumu

Öz

Hepatoid adenokarsinoma morfolojik olarak hepatosellüler karsinomaya çok benzeyen özel bir tip ekstra-hepatik adenokarsinomadır. Hepatoid adenokarsinomaların en sık tespit edildiği yerlerden biri midedir. Karaciğerde büyük kitlesel lezyonlar tespit edilmesi nedeniyle 47 yaşında erkek hasta kliniğimize refere edilmişti. Serum alfa feto-protein düzeyleri çok yüksek saptanan olguda batın ultrasonografisinde karaciğer parankimi sirotik değildi ve viral serolojik göstergeler negatifti. Perkütan karaciğer biyopsisi yapıldı. Biyopsi materyalinin patolojik incelemesinde metastatik hepatoid tip adenokarsinoma metastazı saptandı. Üst gastrointestinal sistem endoskopisinde antrumda ülserler izlendi. Antral biyopsi örneklerinin histolojik incelemesinde epitelial malign tümör tespit edildi.
