Helicobacter pylori-negatif kronik atrofik gastritte karaciğer ve dalak boyutları

Öz

Giriş ve Amaç: Helicobacter pylori-negatif kronik atrofik gastrit, etiyolojisi heterojen olan ve bazı olgularda otoimmün mekanizmaların rol oynayabildiği bir gastrik mukozal atrofi tablosudur. Bu çalışmada, Helicobacter pylori-negatif kronik atrofik gastritli hastalarda karaciğer ve dalak boyutlarına ilişkin bulguların klinik ve laboratuvar verilerle birlikte ele alınması hedeflenmiştir. Gereç ve Yöntem: Retrospektif olarak tasarlanan bu çalışmada, histopatolojik olarak Helicobacter pylori-negatif kronik atrofik gastrit tanısı doğrulanmış 102 hasta değerlendirildi. Hastaların demografik verileri, endoskopik ve histopatolojik bulguları, laboratuvar parametreleri ve eş zamanlı batın ultrasonografi veya bilgisayarlı tomografi raporlarından elde edilen karaciğer ve dalak uzunlukları analiz edildi. Hepatomegali ve splenomegali standart radyolojik kriterlere göre tanımlandı. Bulgular: Hastaların 64’ü kadın (%62.7), 38’i erkek (%37.3) olup tüm kohortun yaş ortalaması 56.08 ± 7.45 yıl idi. Hepatomegali 30 hastada (%29.4), splenomegali ise 4 hastada (%3.9) saptandı. Hepatomegali kadın hastalarda erkeklere göre daha sık gözlenmekle birlikte bu fark istatistiksel anlamlılık düzeyine ulaşmadı (p = 0.068). Hepatomegali varlığı ile yaş, hemoglobin, ortalama eritrosit hacmi, trombosit düzeyi ve intestinal metaplazi arasında anlamlı ilişki saptanmadı. Sonuç: Bu çalışmada Helicobacter pylori-negatif kronik atrofik gastritli hastalarda hepatomegali oranı dikkate değer düzeyde bulunmuş, ancak hepatomegali ile temel laboratuvar parametreleri, intestinal metaplazi ve splenomegali arasında anlamlı ilişki saptanmamıştır. Bulgular, bu hasta grubunda karaciğer boyut artışının olası metabolik veya otoimmün süreçlerle ilişkisinin ileri çalışmalarla araştırılması gerektiğini düşündürmektedir.

Liver and spleen size in Helicobacter pylori-negative chronic atrophic gastritis

Background and Aims: Helicobacter pylori-negative chronic atrophic gastritis is a form of gastric mucosal atrophy with a heterogeneous etiology, in which autoimmune mechanisms may play a role in some cases. This study aimed to evaluate liver and spleen dimensions in patients with Helicobacter pylori-negative chronic atrophic gastritis in relation to clinical and laboratory parameters. Materials and Methods: In this retrospective study, 102 patients with histopathologically confirmed Helicobacter pylori-negative chronic atrophic gastritis were evaluated. Demographic data, endoscopic and histopathological findings, laboratory parameters, and liver and spleen lengths obtained from concomitant abdominal ultrasonography or computed tomography reports were analyzed. Hepatomegaly and splenomegaly were defined according to standard radiological criteria. Results: Of the patients, 64 were female (62.7%) and 38 were male (37.3%), and the mean age of the entire cohort was 56.08 ± 7.45 years. Hepatomegaly was detected in 30 patients (29.4%), whereas splenomegaly was observed in 4 patients (3.9%). Although hepatomegaly was more frequently observed in female patients than in male patients, this difference did not reach statistical significance (p = 0.068). No significant association was found between the presence of hepatomegaly and age, hemoglobin level, mean corpuscular volume, platelet count, or intestinal metaplasia. Conclusion: In this study, the prevalence of hepatomegaly was found to be considerable among patients with Helicobacter pylori-negative chronic atrophic gastritis; however, no significant association was detected between hepatomegaly and basic laboratory parameters, intestinal metaplasia, or splenomegaly. These findings suggest that the potential relationship between increased liver size and metabolic or autoimmune processes in this patient group should be further investigated in future studies.
