Laparoskopik Değerlendirmenin Over Kistleri İçin Tanısal Doğruluğunun Değerlendirilmesi

Öz

Amaç: Over kistleri, kadınlarda sık görülen bir jinekolojik problemdir ve genellikle asemptomatiktir. Ancak bazı durumlarda ağrı, kanama veya infertilite gibi semptomlara neden olabilirler. Over kistlerinin doğru tanısı, uygun tedavi stratejilerinin belirlenmesi için kritik öneme sahiptir. Laparoskopi, over kistlerinin değerlendirilmesinde yaygın olarak kullanılan minimal invaziv bir cerrahi yöntemdir. Bu çalışma, over kistlerinin tanısında laparoskopik ve ultrasonografik değerlendirmenin doğruluğunu histopatolojik bulgularla karşılaştırarak değerlendirmeyi amaçlamaktadır. Gereç ve Yöntemler: Bu retrospektif çalışmada, Ocak 2020 - Haziran 2024 tarihleri arasında Harran Üniversitesi Kadın Hastalıkları Bölümünde laparoskopik cerrahi uygulanmış ve histopatolojik tanı konulmuş tüm adneksiyel kist hastalarının kayıtları incelenmiştir. Hastaların demografik ve klinik özellikleri, ultrasonografik değerlendirmeleri, laparoskopik cerrahi bulguları ve histopatolojik tanı sonuçları hasta kayıtlarından elde edilerek, laparoskopik değerlendirmenin tanısal doğruluğu histopatolojik bulgularla karşılaştırılmıştır. Ayrıca, cerrahın deneyiminin ve kistin derinliğinin laparoskopik değerlendirmenin doğruluğuna etkisi de değerlendirilmiştir. Bulgular: Bu çalışmada, 120 hastanın verileri retrospektif olarak incelenmiştir. Hastaların yaş ortalaması 28,4 ± 7,7 (15-46) yıl olup, kistlerin %45,8'i sol overde, %40,8'i sağ overde ve %13,3'ü bilateral olarak yerleşmiştir. Kistlerin maksimum çaplarının ortalaması 6,8 ± 2,7 (2-18) cm olup, %70'i 5 cm'den büyüktür. Cerrahların %60'ının 5 yıldan fazla deneyimi vardır. Kistlerin %73,3'ü yüzeyel, %16,7'si derin ve %10'u ise yüzeyel ve derin olarak sınıflandırılmıştır. Bulgularımız, over kistlerinin tanı ve tedavisinde laparoskopinin oldukça etkin bir yöntem olduğunu, cerrahi deneyimin önemli bir faktör olduğunu ve deneyimli cerrahlar tarafından gerçekleştirilen operasyonların daha iyi sonuçlar verebileceğini düşündürmektedir. Sonuç: Sonuç olarak, over kistlerinin değerlendirilmesinde ultrasonografi ve laparoskopi gibi yöntemlerin tanısal doğruluğu teyit edilmiş ve cerrahi deneyimin bu kistlerin tanı ve tedavisindeki kritik rolü vurgulanmıştır. Bu bulgu, over kistlerinin yönetiminde tanı ve tedavi süreçlerinin bütünleştirilmesinin önemine işaret etmektedir.

Evaluation of the Diagnostic Accuracy of Laparoscopic Assessment for Ovarian Cysts

Abstract

ABSTRACT Objective: Ovarian cysts are a common gynecological problem in women and are usually asymptomatic. However, in some cases, they can cause symptoms such as pain, bleeding, or infertility. Accurate diagnosis of ovarian cysts is critical for determining appropriate treatment strategies. Laparoscopy is a minimally invasive surgical procedure commonly used in the evaluation of ovarian cysts. This study aimed to evaluate the accuracy of laparoscopic and ultrasonographic evaluation in the diagnosis of ovarian cysts by comparing them with histopathological findings. Material and Methods: In this retrospective study, the records of all patients with adnexal cysts who underwent laparoscopic surgery and were histopathologically diagnosed in the Department of Obstetrics and Gynecology at Harran University between January 2020 and June 2024 were reviewed. Demographic and clinical characteristics of the patients, ultrasonographic evaluations, laparoscopic surgical findings, and histopathological diagnosis results were obtained from patient records, and the diagnostic accuracy of laparoscopic evaluation was compared with histopathological findings. In addition, the effect of the surgeon's experience and the depth of the cyst on the accuracy of laparoscopic evaluation was also assessed. Result: In this study, data from 120 patients were analyzed retrospectively. The mean age of the patients was 28.4 ± 7.7 (15-46) years, with 45.8% of the cysts located in the left ovary, 40.8% in the right ovary, and 13.3% bilaterally. The mean maximum diameter of the cysts was 6.8 ± 2.7 (2-18) cm, and 70% were larger than 5 cm. 60% of the surgeons had more than 5 years of experience. 73.3% of the cysts were classified as superficial, 16.7% as deep, and 10% as both superficial and deep. Our findings suggest that laparoscopy is a highly effective method in the diagnosis and treatment of ovarian cysts, that surgical experience is an important factor, and that operations performed by experienced surgeons may yield better results. Conclusion: In conclusion, the diagnostic accuracy of methods such as ultrasonography and laparoscopy in the evaluation of ovarian cysts has been supported, and the critical role of surgical experience in the diagnosis and treatment of these cysts has been emphasized. This finding indicates the importance of integrating diagnostic and therapeutic processes in the management of ovarian cysts.
