Farklı Burun Tiplerinin Lateral Krural Çalma ve Set Back Kombinasyonu ile Şekillendirilmesi

Öz

Amaç: Tip-plasti, estetik burun ameliyatının sonucu en çok etkileyen bölümlerinden biridir. Farklı deformiteler için farklı teknikleri ve bunların kombinasyonunu kullanma gerekliliği, bu bölümü kimi zaman kafa karıştırıcı ve yorucu hale getirmektedir. Bu çalışmada, tip şekillendirme için lateral krural çalma ve set back tekniklerinin birlikte kullanımı, bu kombinasyonun sağladığı avantajlar, farklı burun tiplerinde kullanım alanları ve sonuçları sunuldu. Gereç ve Yöntemler: 2011 Aralık ile 2017 Aralık arasında açık rinoplasti uygulanan ve tip şekillendirmesinde lateral krural çalma ve set back kombinasyonu uygulanan 50 hasta çalışmaya dahil edildi. Ameliyat öncesinde ve 6 ay sonrasında hastalar ön, yan, bazal ve oblik açıdan fotoğraflandı. Fotoğraflar nazolabial açı, nazal aks, nazal uzunluk ve tip projeksiyonu açısından değerlendirildi. Ameliyattan 6 ay sonra hastalara memnuniyet derecelerini saptamak için; sonuçtan memnun değil, memnun ya da çok memnun olarak anket yapıldı.Bulgular: Tüm hastaların operasyon sonrası altıncı ayda çekilen fotoğrafları, operasyon öncesi çekilen fotoğraflar ile karşılaştırıldı ve amaçlanan hedeflere ulaşıldığı görüldü. Hastalara yapılan anket sonuçlarına göre; 21 kişi sonuçlardan memnun, 15 kişi çok memnun, dört kişi memnun değildi. İki hastaya dorsal düzensizlik nedeni ile, bir hastaya kolumella yanında mukozal fazlalık nedeniyle, bir hastaya medial krus dikişinin oluşturduğu reaksiyon nedeniyle toplamda dört hastaya revizyon uygulandı. Sonrasında hastalarda herhangi bir sorun gözlenmedi. Sonuç: Lateral krural çalma ve set back kombinasyonu farklı burun tiplerine uygulanabilecek pratik bir tip şekillendirme yöntemidir. Rotasyon ve projeksiyon dengesi sağlayan, tip asimetrilerini düzeltmeye imkan veren ve eş zamanlı olarak kolumella sorunlarına çözüm sağlayan çok yönlü bir tekniktir

Lateral Crural Steal and Set Back Combination for Different Nasal Tips

Öz

Objective: Tip-plasty is one of the most influential parts of aesthetic nose surgery. The necessity of combine different techniques for the diversity of nasal tip deformities, sometimes makes this part confusing and complicated. In this study, the combination of lateral crural steal and set back techniques for tip-plasty, the advantages of combination, usage areas for different types of tips and results were presented.Material and Methods: 50 patients who underwent open septorhinoplasty between December 2011 and December 2017 with the technique of lateral crural steal and set back combination are included in the study. Preoperative and 6 months after surgery; patients were photographed from the anterior, lateral, basal and oblique views. Photographs were evaluated for nasolabial angle, nasal axis, nasal length and tip projection. 6 months after surgery, a patient satisfaction survey was done; as not satisfied, satisfied or very satisfied. Results: The photographs of the patients taken at the sixth postoperative month were compared with the photographs taken before the operation and seen that preoperative goals and objectives have been achieved. According to the results of the survey, 21 people were satisfied, 15 people were very satisfied and four people were not satisfied. Four patients underwent revision surgery, two of them were due to dorsal irregularity, one was due to mucosal bulge inside vestibulum and one revision was due to inflamation induced by stitches in columella. There were no problems with the patients afterwards.Conclusion: The combination of lateral crural steal and set back is a practical method which can be applied to different nose tips. It is a versatile technique that provides rotation and projection balance, allows correction of tip asymmetries and simultaneously provides solutions to columellar problems
