Çocuk Yoğun Bakımda Ortalama Trombosit Hacmi Prognozu Etkiler mi?

Öz

Amaç: Ortalama trombosit hacmi OTH trombosit fonksiyonlarını değerlendirmekte kullanılan ve artmış trombosit aktivitesini gösteren önemli bir belirteçtir. OTH’nin yetişkinlerde kardiyovasküler hastalıkların mortalite ve prognozuna ilişkisi üzerine çok sayıda çalışma yapılmış olmasına rağmen çocuk yoğun bakım ÇYB hastalarında yeterli çalışma ve kanıt bulunmamaktadır. Çalışmada OTH ve trombosit sayısının ÇYB hastalarındaki erken dönemde hastalık prognozuyla ilişkisinin değerlendirilmesi amaçlanmıştır.Gereç ve Yöntemler: Ekim 2016-Eylül 2017 tarihleri arasında yatırılan tüm hastaların verileri geriye dönük olarak hasta dosyaları ve elektronik kayıt sistemleri üzerinden incelendi. Bulgular: Toplam 235 hasta çalışmaya alındı. Hastaların 123’ü %52,3 erkek, 112’si %47,7 kız hastaydı. Yaş dağılımları 2 ay ile 17 yaş arasında olup ortalama hasta yaşı 4,02±5,01 yıldı. Serum OTH düzeyi ile mortalite, akut böbrek hasarı, invaziv mekanik ventilasyon IMV , noninvaziv mekanik ventilasyon NIV ihtiyacı, inotrop ilaç kullanımı, sürekli renal replasman tedavisi SRRT ve yoğun bakım yatış süresi arasında istatistiksel anlamlı ilişki saptanmadı. Trombosit sayısı ile NIV, mortalite, akut böbrek hasarı ABH ve SRRT arasında istatistiksel anlamlı ilişki saptandı. IMV, inotrop ilaç kullanımı ve yoğun bakım yatış süresi ile trombosit sayısı arasında istatistiksel anlamlı ilişki yoktu.Sonuç: Ortalama trombosit hacmi ile ÇYB ihtiyacı olan hastaların prognozu ve mortalitesi arasında bir ilişki saptanmamıştır. Trombosit sayısı ise mortalite ile ilişkili bulunmuştur. Sonuçlarımızı destekleyecek daha geniş çaplı ve prospektif çalışmalara ihtiyaç vardır

Does the Mean Platelet Volume Affect the Prognosis in Pediatric Intensive Care?

Objective: Mean platelet volume MPV , an important marker showing platelet activity, is used to evaluate the functions of platelets. Although there are various studies in adults on the relationship between mortality and prognosis in cardiovascular disease and MPV, the number of studies on children is inadequate. We therefore aimed to investigate the association of MPV and platelet count with the early prognosis of patients in the pediatric intensive care unit PICU . Material and Methods: The records of the patients who were hospitalized in our pediatric intensive care unit between October 2017 and September 2017 were investigated retrospectively. Results: A total of 235 suitable patients’ records were evaluated. There were 123 52.3% male and 112 47.7% female patients. The age range was 21 months to 17 years and the mean patient age was 4.02±5.01 years. There was no statistically significant relationship between MPV and mortality, acute kidney injury AKI , invasive mechanical ventilation IMV , non invasive mechanical ventilation NIV , inotropic drug use, continuous renal replacement therapy CRRT , and the duration of hospitalization in PICU. There was a statistically significant relationship between platelet count and NIV, mortality, AKI and CRRT. There was no statistically significant relationship with IMV, inotropic drug use, and the duration of hospitalization in PICU.Conclusion: We did not find any association between MPV and the prognosis and mortality of the patients in PICU. However, the platelet count was associated with the mortality. There is a need for prospective and multicenter studies including more patients to support our results
