Experimental Biomechanical Comparision of Harrington distraction Rod System and Transpedicular Fixator Intern in the Treatment of Dorsolumber Fractures

Harrington distraction rod and transpedicular intemal fixator were compared in the biomechanical experiment. The object of this was to determine the most convenient method in reduction and fixation of dorsolumber fractures. 9 human cadaver spines were subjected to radiographic analysis and 8 of them were determined to be the most convenient biomechanical experiment. The segments between Dorsal 9-lumbar 3 removed and the muscles were cleaned up. In the spines Dorsal 12 vertebral body was osteotomised in all spines and they were mounted to an Instron 1114 testing machine, so that loads could be applied in 15 degrees anterior bending momentum. At the time ofthe testing the chart paper was 20 cm/mnt and loading speed was 10 cm/mnt. Average loading was betveen 20 Nm and 39 Nm (mean loading 27.5. Nm). 4 Dorsolumbar segments which were replaced using the Harrington rods failed at between 21 Nm and 40 Nm. (mean loading 28.75 Nm). And the other spines which were placed by using Internal Fixator falled between 21 Nm. and 39 Nm (mean loading 29 Nm) It was detemined that the breakage resistance of the spines, having Harrington rods, before the application of the instrument was about 28 Nm and the one after the application of the instrument was 28.75 Nm. However the first breakage resistance of the spines with Internal Fixator was 27 Nm and the breakage resistance of the spines after the application of the Fixator Intern was 29 Nm. According to these findings the breakage resistance of the spines with Internal Fixator under the anterior bending momentum was found to be 1.25 Nm better than the breakage resistance of the spines having Harrington rods. The difference was determined to be rather less than the one found in the references. The failure occured in the bones where instruments had been placed, not in the instruments themselves. The results closely related to the resistance of the bones.
