Comparison of the forty-five degree postero-anterior flexion and the antero-posterior extension weight-bearing radiographs

The most important radiographic finding of gonarthrosis is joint space narrowing. This study attempted to compare the forty-five degree postero-anterior flexion weight-bearing radiograph and the conventional radiograph as regards their ability to detect joint space narrowing, and to reveal the grade of arthrosis. 47 knees of 25 patients who had both radiographs were included in the study. The patients' ages ranged between 39 and 72 years (av. 59 years). The grade of articular cartilage Iesions could be noted in 10 knees of 10 patients. Radiographic grading was done according to Ahlback. The medial compartment was av. 1.4 mm narrower and the Iateral compartment was av. 0.8 mm narrower on the flexion weight-bearing radiographs as compared to the conventional radiographs. 75% 0' the medial compartments and 57% of the Iateral compartments were narrower on the flexion radiograph. With the flexion radiographs, the degree of arthrosis was I-II grades higher for the medial compartment in 64% and for the Iateral compartment in 28%. Flexion radiographs correlated better with the degree of cartilage Iesions but the number of cases was too small to make a conclusion in this regard. it is concluded that, the forty-five degree flexion weight-bearing posteroanterior radiograph of the knee should be routinely obtained before a treatment plan is made in cases of gonarthrosis.
